The respect for autonomous choices of person runs as basic in Modern medicine. But the Autonomy has so deeply involved in the multi-culture movement of society, that more and more conflicts between patient demands and medical ethics are coming up. These medical staff have to face a difficult choice of medical treatment, especially in China, the confucianists culture, people would take the family as the core. Although we have laid down a set of rules about respect for autonomy, the decision-making of patients in China have to be limited by their families. There are already many unexpected medical disputes which caused from this imperfect decision model. So, the clinical decision making model combined patient and their family might be fit to china better. And we will discuss as belowFirstly, we make a clear definition for the principle of respect for autonomy in Bioethics:autonomy in mind, in will, in act, which is a contrast to the tradition collectivism in China. Autonomy for doctor and autonomy for patient get their owe distinguishing features. However, due to some typical Chinese traditional culture such as "family based", the rights for autonomy shows some particularity. It’s not suitable to run the model locally which totally on the patient or the family. Only the model of "based patient, involved family" could take some effect.Secondly, to analyze models of patient-decided and family-decided. In China, even though patient-decided model show totally respect for the patient, it would hurt their family in economy and emotion. And the family-decided model may do some harm to the rights and health of patients. So what we should do is to allow the family to make some decision in occasion, which is a kind of limited right to protect the patient better.Last, to design some strategies to solve the problem. As needed, to figure out the principles to solve the conflicts between patient and their family. And followed by these principles, we lay down some rules about ethics, laws and use different way to make decision in different situations.
