| Objective:To study the parameters and morphological characteristics of the proximal femur of Chinese central south region population, provide a basis for Chinese femoral prosthesis design and a reference for surgeons to select the prosthesis.Methods:AP and lateral radiography of the hip joint were performed using Philips Digital Diagnost DR radiography system in the Second Xiangya Hospital radiology department. The shooting range covered the hip joint and at least2/3of proximal femur. Excluding the patients with obvious hip disease,500patients were enrolled in this study. Using Onis2.3software various parameters were measured which included the diameter of femoral head, neck shaft angle, femoral offset, the medullary cavity diameter at the level of20mm above the lesser trochanter center (T+20), the medullary cavity diameter at the level of the lesser trochanter center (TO), the medullary cavity diameter at the level of20mm below the lesser trochanter center (T-20), the isthmus width, extracortical width at isthmus level, the location of the isthmus, the femoral canal flare index (CFI), the metaphyseal canal flare index (MCFI), the distal canal flare index (DCFI), cortical index (CI), Noble type, Dorr type. After measurement the data were compared with the studies based on the west people and another domestic study, and we analyzed whether there were differences between left and right side, between men and women. The correlations between the parameters and age were analyzed. Nolbe and Dorr classification methods were used to analyze the canal shape of the proximal femur.Results:1. The femoral head diameter is44.21±3.75mm, neck shaft angle is128.29±5.74°, offset is38.82±5.99mm, T+20is43.55±5.26mm, TO is25.47±3.60mm, T-20is18.32mm±2.98mm, the internal diameter of the isthmusn is10.71±2.26mm, the outer diameter of the isthmus is27.42±2.80mm, the location of the isthmus is106.97±15.92mm, CFI is4.24±1.00, MCFI is2.42±0.35, DCFI is1.75±0.31, cortical index is0.61±0.07.2. There is no significant difference between the of left and right side.3. Compared with the west people, the offset is significantly smaller, while the neck shaft angle is significantly larger, the most parameters of the proximal femoral medullary cavity diameter is significantly smaller than those in white peoples.4. Compared with the west people, the CFI is significantly larger. According to the Noble classification, the proportion of champagne flute type is significantly larger, while the proportion of standard type is significantly smaller. The consistency test between the Noble classification and the Dorr classification is statistically significant, and the consistency is general.5. Compared with another domestic study, some of the parameters are significantly different. However, the practical significance is poor because of the small numerical difference.6. Several parameters of proximal femur are relevant to age.7. The differences of several parameters between male and female are statistically significant.Conclusion:Proximal femoral parameters of Chinese central south region population are significantly different compared with the west people. Several parameters are affected by age and gender factors. These factors should be concerned when designing and choicing the prosthesis. Prosthesises with a variety of shapes and dimensions should be designed to meet the requirement of the patients. |
