The Analysis Of Clinical Features In The1360Tuberculosis Children Cases

Objectivc::Analyse the datum of tuberculosis infected children around ChongQing in our hospital, to summarize differents of all types of tuberculosis infection, to improve the consciousness of diagnose and the methods, and lower down the misdiagnosis rate then sum up the therapeutic effect of our hospital.Method:Do retrospectiveanalysis of1360cases from2003to2010, learn about the popular、symptom、physical、Laboratory Examinations etc. Results:1.Boys (58.8%) are much more than girls (41.2%),0-2years and11-13years are the mainly ages,the mean age of boys are6years old,while girls are6.8years,which has significance differents.The patients in the countryside are more than in the cities and towns,38.8%have clearly contact history,most happened in the lineal relative.2.BCG (Bacillus Calmette-Guerin)can effectively low down the incidence of hematogenous pulmonary tuberculosis and the tuberculous meningitis.3.The positive rate of TB antibody IgG is10.8%,the positive rate of Mycobacterium tuberculosis-PCR is25%.sputum smear bacilli positive rate is40.6%,and the culture positive rate is22.5%,during the Biopsy cases91.6%can make the diagnose of Tb.4.Iconography:thorax changes happen mostly in right;brain CT shows the ventricle enlarging(55.1%),hydrocephalus(49.7%),basal ganglia changing(42.2%) are the common signs,also with the combine up signs.5.For the first in-hospital patients,the pulmonary tuberculosis are832cases(73.7%),primary pulmonary tuberculosis is31.6%,which is the maximum type,then are the secondary pulmonary tuberculosis(27.6%) and the tuberculous pleurisy(27.6%), the last is the hematogenous Pulmonary Tuberculosis(25.2%).6.During the extrapulmonary tuberculosis (EPT),the percent of tuberculous meningitis is38.3%, abdominal tuberculous is12.6%, tuberculous lymphadenitis is5.0%, osteoarticular tuberculosis is10.2%, urinary system tuberculosis is2.0%, endobronchial tuberculosis(EBTB) is1.2%.7.For the hematogenous pulmonary tuberculosis, the mean age is0-6months old, for the secondary pulmonary tuberculosis is no different, for the pulmonary tuberculosis is older than1years old, for the tuberculous meningitis is6months-3years old, for the osteoarticular system tuberculosis is1-6years old. and for the other types, the mean ages are both12-18years old.8.At the same time with the regular anti-tuberculosis Chemotherapy treatment, tiny dose of steroid can be helpful for curing the hematogenous pulmonary tuberculosis, the tuberculous pleurisy, the tuberculous meningitis and the abdominal tuberculous.Conclusions:The clinical analysis of TB in our hospital are almost same with other repots, but the incidence of the tuberculous meningitis is much more than others, while the osteoarticular tuberculosis is less than other dotas. The improvement rate of all TB patients is70.5%, but the mortality of the tuberculous meningitis is high(6.0%). It’s still severe that children’s tuberculosis can’t be found easily,so we should comprehensive all the clinical data,even do the diagnostictreatment.
