Objective:The objective of this study is to determine the levels of blood glycosylated hemoglobin, interleukin-6and adiponectin in patients with coronary heart disease, exploring the relation between they and coronary heart disease and coronary arteriosclerosis changes. Further, we analyze their mechanism in the formation and development with coronary heart disease so as to give new idea and method about prevention ang treatment in the coronary heart diseaseMethod:88patients were enrolled in our study who underwent the coronary Angiography from Novermber2010to June2011in Department of Cardiology of our hospital,and were confirmed to have or not CHD. The patients were divided into coronary heart disease group(n=66) and control group(n=22)according to the Coronary Angiography (CAG). CHD group was further divided into stable anginapectoris(SAP)group(n=23), unstable anginapectoris(UAP)group(n=21)and acute myocardial infarction(AMI)group(n=22)according to the clinical property. Meanwhile CHD group was divided into single-branches group, double-branches group and multi-branches group according to the number of coronary artery stenosis. The severity degree of coronary artery stenosis was quantitively assessed according to coronary angiography by Gensini score. They were divided into four groups according to Gensini score:normal control group(Gensini score=0); light stenosis group(1≤Gensini score≤20); moderate stenosis group(20<Gensini score<40)and severe stenosis group(Gensini score≥40). Glycosylated hemoglobin Alc was measured by ion exchange chromatography, IL-6and APN by enzyme-linked immunosorbentassay. Experimental data were processed by SPSS17.0statistical analysis software. A value of P<0.05was considered statistically significant.Result:1The comparison of HbAlc level:(1)The HbAlc levels of CHD group were higher than the control group[(6.93±0.98)%vs (5.51±0.44)%, P<0.01],(2) In CHD group, as the number and severity of coronary artery stenosis increased, the HbA1c level also increased. The HbA1c levels of single-, double-, muti-branches group were (6.40±0.68)%,(7.27±0.46)%,(8.17±0.91)%; The HbA1c level of light stenosis group, moderate stenosis group and severe stenosis group were (6.36±0.42)%,(7.48±0.41)%,(8.43±0.81)%,(3) The HbA1c level of SAP group were higher than that in normal control group [(6.35±0.45)%vs (5.51±0.44)%, P<0.01]; The HbA1c level of UAP group were higher than that in SAP group [(7.23±0.66)%vs (6.35±0.45)%, P<0.01]; The HbA1c level of AMI group were higher than that in UAP group [(7.26±0.88)%vs (7.23±0.66)%, P>0.05]2The comparison of IL-6level:(1)The IL-6levels of CHD group were higher than the control group[(34.76±6.07)pg/ml vs (22.50±1.23)pg/ml, P<0.01],(2) In CHD group, as the number and severity of coronary artery stenosis increased, the IL-6level also increased. The IL-6levels of single-, double-, muti-branches group were (28.42±5.25)pg/ml,(33.53±4.06)pg/ml,(37.71±5.43)pg/ml; The IL-6levels of light stenosis group, moderate stenosis group and severe stenosis group were (28.27±4.23)pg/ml,(34.05±4.57)pg/ml,(39.80±2.89)pg/ml,(3) The IL-6levels of SAP group were higher than that in normal control group [(26.37±1.38)pg/ml vs (22.50±1.23)pg/ml, P<0.01]; The IL-6levels of UAP group were higher than that in SAP group [(37.53±2.75)pg/ml vs (26.37±1.38)pg/ml, P<0.01];The IL-6levels of AMI group were higher than that in UAP group [(40.88±3.24)pg/ml vs (37.53±2.75)pg/ml, P>0.05]3The comparison of APN level:(1)The APN levels of CHD group were lower than the control group[(6.20±1.05)μg/ml vs (10.25±0.77)μg/ml, P<0.01],(2) In CHD group, as the number and severity of coronary artery stenosis increased, the APN level also decreased. The APN levels of single-, double-, muti-branches group were (7.29±0.85)μg/ml,(5.62±0.52)μg/ml,(4.78±0.86) μg/ml; The APN levels of light stenosis group, moderate stenosis group and severe stenosis group were (7.11±0.96)μg/ml,(5.72±0.75)μg/ml,(4.39±0.28)μg/ml,(3) The APN levels of SAP group were lower than that in normal control group [(7.60±0.22)μg/ml vs (10.25±0.77)μg/ml, P<0.01]; The APN levels of UAP group were lower than that in SAP group [(5.46±0.76)μg/ml vs (7.60±0.22)μg/ml, P<0.01]; The APN levels of AMI group were lower than that in UAP group [(5.44±0.28)μg/ml vs (5.46±0.76) μg/ml, P>0.05]4Pearson correlation analysis showed that HbAlc concentrations were positively correlated with IL-6(r=0.808, P<0.01) and were negatively correlated with APN(r=-0.794, P<0.01) and that IL-6concentrations were negatively correlated with APN(r=-0.794, P<0.01). In addition, Gensini score were positively correlated with HbAlc and IL-6(r=0.557, P<0.01; r=0.426, P<0.01), and negatively correlated with APN(r=-0.395, P<0.05). Stenosis vessels were correlated with them.Conclusion:1HbAlc, IL-6and APN are associated with CHD, APN may be a protected factor of CHD. HbAlc and IL-6may be promote the formative process of coronary atherosclerosis.2HbAlc, IL-6and APN are significantly associated with coronary lesion complexity in patients with CHD. HbAlc and IL-6may be promote the progression of coronary artery changes. Hypoadiponectin may be attenuating the progression of coronary artery changes.3Gensini score are positively correlated with HbAlc and IL-6, and negatively correlated with APN. Thus monitoring their level may be effective an index to evaluate the coronary artery change degree.
