Objective:Using color Doppler ultrasound technology to measure homodynamic parameters of Superior Thyroid Artery、Inferior Thyroid Artery、 External carotid artery in thyroid solid nodule patients, calculate The mean of blood flow velocity (Vmean)、Blood flow(Q) in above three vessels and the Ratio of the blood flow (R) between Superior Thyroid Artery and External carotid artery, to explore the clinical significance of the qualitative diagnosis among all kinds of solid thyroid nodules through the changes in the R-value(thyroid artery steal blood phenomenon)Materials and Methods:61cases of thyroid solid nodules patients were studied, in which21cases were malignant40cases were benign. selected40cases of the same period in check without thyroid disease and other endocrine and metabolic abnormalities healthy individuals as control group.All patients underwent two-dimensional and color Doppler ultrasound scanning, all cases were confirmed by pathology. Using color Doppler ultrasound diagnostic instrument to measure homodynamic parameters (including inner diameter and flow velocity) of Superior Thyroid Artery、Inferior Thyroid Artery External carotid artery in each study group, and according to the calculation formula Vmean=1/2(Vs+Vd)、Q=1/47πD2Vmean60and R=QSTA/QECA×100%, calculate The mean of blood flow velocity、blood flow volume per minute in three vessels.and the Ratio of the blood flow between Superior Thyroid Artery and External carotid artery. Using statistical methods to focus on the significance of the R-value and each of these homodynamic parameters to identify the qualitative of thyroid nodules.Results:1、In benign and malignant thyroid nodules group, the inner diameter、 The mean of blood flow velocity、the blood flow volume per minute of STA、ITA in nodule side are higher than non-nodule side (P<0.05). Of the STA、ITA, the inner diameter of The mean of blood flow velocity, the blood flow volume per minute thyroid malignant nodules group> benign nodules group> the control group.(P<0.05).2、This study shows that the ECA blood flow in Benign and Malignant Thyroid nodules group comparison between ipsilateral and healthy control group, bilateral difference was not statistically significant (P>0.05).3、STA and ECA blood flow ratio R between the two groups and within-group comparison shows that40cases of benign nodules ipsilateral R=8.5±1.64, contralateral R=5.3±1.21;21cases of malignant nodules ipsilateral R=16.1±2.43, contralateral R=10.3±1.98. Benign and malignant nodules health, ipsilateral R value difference was statistically significant (P<0.05), the R-value of the malignant nodules was significantly higher than benign nodules. Through statistical analysis of this group of patients, found that the minimum R=10false-negative rate and false positive rate can be used as the optimal threshold, R<10the possibility of a diagnosis of benign nodules, when R>10,the possibility of a diagnosis of malignant nodules, R=10is the critical value, the corresponding accuracy of88.5%, sensitivity85.7%and a specificity of90%.Conclusion:When solid nodules appear in Thyroid, the blood flow of the Superior Thyroid Artery in benign and malignant nodules groups has increased to varying degrees, It can be called Superior Thyroid Artery steal phenomenon.the blood flow ratio (R value) between the Superior Thyroid Artery and the External carotid artery in nodule side is objective indicators on the evaluation of the Superior Thyroid Artery steal degree, and It has great clinical significance in the initial diagnosis of benign and malignant thyroid nodules.
