ObjectiveSpontaneous abortion(spontaneous abortion, SA) is one of the pregnancy common complication, accounting for10%-15%of all pregnancy, its etiology is complex and the pathogenesis is unclear. Studies suggest that SA patients with thrombophilia at home and abroad. The imbalance expression of uPA and PAI-1can lead to easy thrombospondin and also Impact the ability of trophoblast invasivison. In the physiological processes, uPA can promote endometrial stromal degradation,which play an important role for trophoblast invasivison and the growth of the fetus implantation. PAI-1can inhibit the activity of uPA,Which regulate the plasminogen-induced extracellular matrix degradation, cell invasion and angiogenesis. UPA/PAI balance regulate fetal perfusion and trophoblastic cultivation and invasion, Once this balance is destroyed, it will affect the villi of the perfusion of trophoblast invasion, resulting in insufficient blood supply to the fetus and cause SA. plasma PAI-1, uPA gene and protein expression abnormalities related to the occurrence of the SA at home and abroad.But there has not been enough relevant information reported about PAI-1DNA polymorphism, PAI-1protein and uPA expression in the chorionic villi in spontaneous abortion situation. In this study, the expression of uPA and PAI-1gene and protein in the chorionic villi are tested,In order to provide the mechanism of laboratory evidence for spontaneous abortion.MethodsSelect30cases of normal early pregnant women to terminate unwanted pregnancies requirements for the control group and the same period in diagnosis of patients with missed abortion30cases for the experimental group.they are all treatment in the obstetrics and gynecology clinic of the First Affiliated Hospital of Zhengzhou University. Rinse the villi clean and suck up the water under sterile conditions, part in DEPC soaked EP tube and immediately placed in-80℃refrigerator to save spare; another part formaldehyde solution by the volume fraction of10%and fixed overnight then embedded in paraffin, stored in-20℃refrigerator to save spare. Immunohistochemical method(PV method) is used to detected the PAI-1and uPA protein distribution useing f the part of embedded. PAI-14G/5G polymorphism Is detected by allele-specific-PCR (AS-PCR).and the uPA mRNA expression Is detected by RT-PCR, their specimen are all using the part at the-80℃refrigerator.Results1. During the pregnancy, the main expression of PAI-1protein is in the the cytoplasm of trophoblastic villi,And the expression rates of PAI-1in spontaneous abortion group (23/30,90.00%) is higher than the normal pregnancy group (5/30,16.67%,), the difference was statistically significant (X2=32.411, P<0.05).2. During the pregnancy, the main expression of uPA protein is in the the cytoplasm of trophoblastic villi,And the expression rates of uPA in spontaneous abortion group (5/30,16.67%) is lower than the normal pregnancy group (26/30,86.67%), the difference was statistically significant (X2=29.433, P<0.05)).3. The AS-PCR results of PAI-14G/5G gene polymorphismFor the reference of the5G/5G,4G/4G genotype in the spontaneous abortion group is higher than the normal pregnancy group, the difference was statistically significant (X2=3.901, P<0.05); The Odds ratio is4.000and the95%Confidence interval is (0.981-16.311).4G allele in the spontaneous abortion group is also higher than the5G allele, the difference was statistically significant (X2=4.848, P<0.05), The Odds ratio is2.27and the95%Confidence interval is (1.088-4.733).4. The RT-PCR results of uPA mRNAuPA in the spontaneous abortion group and the normal pregnancy group,:0.2230.018,0.5050.059, spontaneous abortion group than in normal pregnancy group, the difference was statistically significant (t1=14.349, P<0.05). the average gray value of the uPA in the spontaneous abortion group and the normal pregnancy group are0.223±0.018,0.505±0.059, spontaneous abortion group is lower than the normal pregnancy group, the difference was statistically significant (t1=14.349, P<0.05).5. Associated analysis From pearson correlation analysis found that the PAI-1and uPA protein expression was negatively correlated in the spontaneous abortion patients villi and their correlation coefficient is r=-0.639.Conclusions1. The low expression of uPA and the high expression of PAI-1and PAI-14G/5G polymorphism in the chorionic villi all related to the spontaneous abortion.2. The expression of uPA and PAI-1in the chorionic villi imbalances related to spontaneous abortion. |
