Objective By the material collected from in months from March to April, this study analyzed the Prevalence, status of current awareness, treatment of diabetes and the economic burden of diabetes (the direct economic burden, indirect economic burden and invisible economic burden) among residents aged18years and over from Luo Ping county; To probe the social-economic factors of diabetes, and to analyze the factors of the disease economic burden.Methods Probability proportional to size (PPS) sampling method was used to select representative sample of4,801residents. over from Luo Ping county. Information was obtained from a cross-sectional health examination survey. Two-steps method, disability adjusted life years (DALY) combined with human capita, and willingness to pay was used to measure direct, indirect and intangible economic burden of diabetes, respectively. the economic burden of diabetes is the cumulative result including the direct,indirect and intangible economic burden. The occurrence of individual diabetes or not was used as the dependent variable, the socio-economic indicators of individuals and groups was used as the independent variable.And the individual economic indicators were the first level, the economic indicators of townships or village were the higher level. The establishment of two-level model is to analyze the factors of diabetes. Multiple linear regression was applied to analyze possible influencing factors of economic burden of diabetes.Results Among the study populations, the overall standardized prevalence of diabetes was6.79%, and females had higher standardized prevalence of diabetes than males (7.95%vs.5.27%, p<0.05). Prevalence of diabetes increased with age, whereas decreased with level of education (P<0.05); The prevalence of different marital status were also different, and the widowed group had the highest prevalence; Prevalence of the alcohol group was higher than the non-drinking group(P<0.05), and the smoking group had higher prevalence of diabetes than the non-smoking group(P<0.05); Prevalence of the central obesity group was higher than the non-central obesity group(P<0.05), and the obesity group had higher prevalence of of diabetes than the non-obesity group; There was no difference between the prevalence of high-salt diet group and that of low-salt diet group(P>0.05). There was no significant difference of prevalence of diabetes beteen Han and the Minority(P>0.05).The awareness and treatment of diabetes was9.8%and8.5%respectively, and the awareness rate is higher than treatment of diabetes(P<0.05). Adults with higher cultural degree had higher probability of having lower prevalence of diabetes(P<0.05); The rate of awareness and treatment of45-year-old group were higher than the other aged groups (P<0.05); There was significant difference among the cultural degrees (P<0.05). There was no significant difference of awareness and treatment of diabetes beteen Han and the Minority (P>0.05).By establishing two-level model to analyze, older adults had higher probability of occurrence of diabetes(OR=1.55,95%C1:1.21~1.99); females had higher probability of occurrence of diabetes than males(OR=1.04,95%CI:1.02~1.05);The towns with high proportion of Minority had lower probability of occurrence of diabetes than the towns with low proportion of Minority(OR=0.98,0.96~0.99).The YLL/1,000population of diabetes was0.37, and0.36for males, and0.39for females; The YLD/1,000population of diabetes was2.25, and1.75for males, and2.67for females; The DALY/1,000population of diabetes was2.62, and2.11for males, and3.06for females. The per capita direct, indirect and intangible economic burden of diabetes was8318.61Yuan,26.2Yuan and8370.00Yuan, respectively,and The per capita economic burden of diabetes was16714.81yuan. The total economic burden of diabetes was637,463,800Yuan, which was the direct economic burden and the intangible economic burden mainly; The per capita males, females economic burden of diabetes was20624.55yuan and15094.52yuan respectively, and the economic burden of males was higher than that of females(P<0.05).The length of hospital stay was one of factors of economic burden of diabetes.Adults with longer length of hospital stay had higher probability of having higher economic burden of diabetes.Conclusion The local rural residents had higher prevalence of diabetes, but the rates of awareness and treatment of diabetes was low, and diabetes has caused a heavy economic burden. These findings suggest that diabetes has been one of main diseases which affected the health of the local residents, so the local health administrative departments should improve the fairness of health services for rural residents, especially the poor residents. To invest more health resources to the township hospitals and village clinics, so that more workers of qualified primary health go to rural areas to propaganda the knowledge of diabetes prevention, so as to improve health awareness.To control high medical costs and hospitalization days of diabetes is one of the effective means to reduce the economic burden of diabetes. Examination of diabetes is as a routine examination, so that people with diabetes can do "early discovery, early diagnosis and early treatment", and to control the development of diabetes and to prevent complications of diabetes.
