OBJIECTIVE This study estimated the prevalence and economic burden of mainly respiratory system disease(COPD and asthma), we measured the direct,indirect and intangible economic burden and also calculated the influencing factors for the burden of Guan du district in Kunming.The results would provide some suggestions or evidence for the local relevant departments to prevent and control the disease.METHODS We selected Guan Du district which consisted of9townships as the study community. Three villages was chosen from each village by the probability proportional to size (PPS).In each selected village, we obtained a name list of individuals aged>18years from the village committee, and random sampling was used to select200individuals from each village. Information was obtained from a cross-sectional study based on questionnaire survey. Two-steps method was used to measure direct economic burden of COPD and asthma, indirect economic burden of COPD and asthma was calculated by disability adjusted life years (DALY) combined with human capita method, and intangible economic burden was estimated by willingness to pay method. Multiple linear regression was applied to analyze possible influencing factors of economic burden of COPD and asthma.Data analyses were done with SPSS software version11.5.RESULTS There were a total of4595people in this study, including2167male(47.2%) and2428female(52.8%), nationality was4365, accounting for95.0%, minority was200, accounting for4.4%. About37.6%of our target population was45-59years old, most of them just graduated from primary school or middle school, and families with per capita income was about5200yuan. In the selected village, we got about4595received questionnaire, with a questionnaire response rate of98.5%. The overall prevalence of asthma and COPD were1%,1.3%, respectively. Among males, the prevalence of asthma and COPD was1.2%,1.2%, it was0.9%,1.4%for females. There was no significant difference between males and females of both of the tow disease.For the patients with asthma, different cultural level had different incidence (p<0.01), lower education level had higher prevalence.Both of the two disease,the prevalence increased with age,the older group had higher prevalence than the younger ones. Smokers had higher prevalence than non-smokers (p<0.05). Drinkers had higher prevalence than non-drinkers (p<0.05). The incidence of asthma was higher between overweight individuals and central obesity people. The obesity ones and the central obesity ones had higher prevalence of COPD than the normal people.The per1000population DALY for COPD, asthma was14.99,0.87, respectively. COPD was more harmful for people's health.The total economic burden of COPD is1,733,620,000yuan, the per capita direct, indirect and intangible economic burden of COPD were3750.42yuan,8801.5yuan and7763.04yuan. The total economic burden of asthma is8532.97yuan, the per capita direct, indirect and intangible economic burden of COPD were5351.49yuan,400.15yuan and1940.00yuan. In both of them, medical fee accounted for the most part in direct economic burden.The patients of COPD's financial burden mainly influenced by the hospital level, higher level, higher burden; patients suffered from asthma with longer length of being in hospital and harder payment had higher probability of having higher direct economic burden.CONCLUSION After the investigation, we found that compared with the national level the incidence of the main respiratory system disease (COPD, asthma) in Guan Du region in Kunming was lower, but we could not overlook it, in contrast,we must be more concern about it. Further more, the economic burden of the main respiratory system disease here is heavier, which was more prominent in the relevant research. Adequate attention concerned should be given by the official departments or other correlated units and actively measures should be taken to prevent its further development.
