Objective: Irritable bowel syndrome is a kind of disorders of intestinalfunction, and its incidence rate is very high. In our country, the proportion ofpatients with IBS in gastrointestinal clinic treatment is20%-50%. IBS-D is themost common type, accounted for74%of the total cases. Currently, mostscholars consider that the disease lacks Morphological changes and (or)biochemical abnormalities which can explain the symptoms. In westernmedicine, symptomatic treatment is the main treatment method, they useantispasmodic, antidiarrheic, prokinetic agents, antidepressant, visceralanalgesic,gastrointestinal probiotics,have a certain effect, but also haveadverse reaction and a high rate of recurrence. Traditional Chinese medicineresearches this disease with its unique etiology and pathogenesis theory, usesdecoction to treat this disease, regulates comprehensively, and achieves goodcurative effect. In clinical practice, we use Si Ni Tong Xie decoction to cureIBS-D, and obtain satisfactory results, so we want to discuss the mechanismof action of this decoction from animal experiment, provide objective basis forits clinical application, and explore a new way to cure IBS-D.Methods:100Wistar male rats were randomly divided into5groupswith20rats in each group: the normal group (N group), model group (Ngroup), the Pinaverium bromide group (P group), the high-dose SNTX (HDgroup), the low-dose SNTX (LD group). In addition to the normal group,other four groups were create IBS-D model by the united way: gastric lavagewith Fanxieye,2ml/one, once a day, last two weeks. Two weeks later, afterone hour of gavage, clamped their tails to make them fight,30minutes onetime, once a day, last two weeks. After completing model, killed the rats inmodel group, obtained blood samples and colon samples, stored aftertreatment, for observing histopathological change and test parameters at last. From the second day, rats in other groups were lavaged with drug. The normalgroup was fed with2ml distilled water, two groups were lavaged with high,low dose of Sinitongxie decoction (37.5g/kg,12.5g/kg), the P group waslavaged with Pinaverium bromide(50mg/kg),2ml at a time, once a day, last14days. We observed the situation of excrement, activity, pelage, eating, etc.They were killed at24hours after the last administration, each was collectedblood sample to be assayed the contents of5-HT and IL-1β. We dissected therats immediately, took3samples of colon (1cm), routinely dehydrated,embedded in paraffin, stained with HE, observed under a microscope, anddetect the content of VIP in the colonic tissues by immunohistochemistry. Thedata of tats was analyzed with SPSS13.0statistical software.Results:1The general stateCompared with the normal group, six day later, the molding ratsemergeed loose stools, even water sample stools, increased defecation, ate less,fur lackluster, lacked of activity and spirit, reduced weight. After treatmentwith high-dose SNTX and Pinaverium bromide, the rats' fur and excrementbecome normal, food intake, weight and activity increase more. The generalstate of the rats in the low-dose SNTX group improved in a certain extent, butless than the high-dose SNTX group.2The observation of colon organizationVisual observation: The luster of the colon mucosa is normal, and thereis no ulcer and erosion on it.Light microscopic observation: The mucous membrane of colon iscomplete, with no obvious congestion and hydroncus. The gland arrangementis neat. The submucosa and muscularis is normal.3The content of5-HT,IL-1β,VIPThe5-HT content in M group is higher than N group obviously (P<0.05).After the treatment, the content in HD group, LD group and P group is lowerthan in M group (P<0.05). The content in HD group and P group is lowerthan in LD group (P<0.05). There is no significant difference between the HD group and the P group (P<0.05).The content of IL-1β in serum and the content of VIP in colonic tissue inM group is higher than N group obviously (P<0.05). After the treatment, thecontent in HD group, LD group and P group is lower than in M group (P<0.05). The content in HD group is lower than in LD group and P group, thedifference has statistics significance (P<0.05).Conclusion:1Sinitongxie decoction can improve the general state of the IBS-D rats.After treatment with high-dose SNTX and Pinaverium bromide, the rats'general state is improved obviously. The effect of the low-dose SNTX isinferior to the high-dose SNTX.2The high-dose SNTX, low-dose SNTX and Pinaverium bromide canreduce the content of5-HT and IL-1β in the serum and the content of VIP incolonic tissue. Sinitongxie decoction can make the power and sensitive andthe water and electrolyte metabolism of the intestine become normal, recoverthe function of gastro-intestinal tract, cure abdominal pain and diarrhea. Ourexperiment show that Sinitongxie decoction has good effect on the rats ofIBS-D.
