Objective To study the relationship between carotid atherosclerosis and C-reactive protein, leptin and adiponectin in patients with metabolic syndrome(MS) and to explore the potential mechanism of formation and progress of atherosclerosis in such patients, and to analysis multivariately the interaction between inflammatory factors and adipokines, and the mechanism to cause insulin resistance and atherosclerosis. It was useful for us to make further clinical significance for the clinical prevention, diagnosis and treatment of metabolic syndrome and atherosclerosis.Methods According to the findings of bilateral carotid artery echocardiography,226 patients with MS were divided into no carotid artery plaque group (MS-1 group, n=94) and carotid artery plaque group (MS-2 group, n=132). Thirty-six healthy subjects were served as normal controls. Serum insulin, high sensitivity C-reactive protein (hs-CRP), leptin(Leptin), adiponectin(APN) were examined and bilateral carotid artery ultrasonography was perdormed to detect plaque formation. Logistic regression analysis was used to determine independent predictors for carotid atherosclerosis MS patients.Results 1. Carotid artery intima-media thickness (IMT), insulin resistance index (HOMA-IR), hs-CRP, Leptin were significantly higher and APN was significantly lower in MS group than those in control group (both P <0.01).There were also significant differences in above parameters between MS-1 group and MS-2 group (P<0.01).2. IMT was positively correlated to HOMA-IR, hs-CRP and Leptin (all P<0.01), and negatively correlated to APN(P<0.01). hs-CRP, APN and Leptin were independed predictors for IMT.Conclusions C-reactive protein, leptin and adiponectin were correlated to carotid atherosclerosis in patients with MS, which may be used for risk stratification in MS patients.
