Investigation On Macrocytic Anemia Related Diseases And Its Clinical Correlation

Objective:To discuss Macrocytic anemia(MCV>95fl)blood system related diseases and its clinical significance.Methods:159 cases of macrocytic anemia were found when the inpatients'blood that was collected when they first checked in the hematology of Da Lian Central hospital from January 1 2008 to September 30 2010 by XE2100,an automatic complete blood corpuscle analyzer. We collected the patients'detailed medical history and physical examinations,reticulocyte count,peripheral blood and bone marrow morphology,liver function test and thyroid function test.Some of these patients did the examination of chromosomej,fusion gene and flow cytometry. We made the etiological diagnosis based on all the information above.Results:We finally found that among the 159 cases of macrocytic anemia, 46 cases were Megaloblastic anemia,43 cases were Leukemia , 21 cases were Myelodysplatic syndrome,15 cases were all pancytopenia,10 cases were Multiple myeloma,9 cases had Autoimmune pancytopenia,9 cases had Autoimmune hemolytic anemia ,3 cases had Primary myelofibrosis,2 cases were Evans syndrome,1case were Paroxysmal nocturnal hemoglobinuria.And Megaloblastic anemia,Leukemia, Myelodysplatic syndrome,aplastic anemia,Multiple myeloma,in these five diseases,the percentage of RDW-CV value>15% respectively were 97.8%,86.0%,85.7%,80.0%,60.0%,indicating that most of cases in the five diseases were red blood cell heterogenecity type of anemia.Autoimmune pancytopenia of RDW-CV value>15% were 44.4%,indicating its cases have more homogeneous red blood cell anemia。But all of Autoimmune hemolytic anemia,s RDW-CV value overtook 15%,so all of them were red blood cell heterogenecity type of anemia,which might be caused by relatively small sample size. Conclusions:1.The common diseases of the macrocytic anemia are Megaloblastic anemia,Leukemia,Myelodysplatic syndrome, and most of them were red blood cell heterogenecity type of anemia.2.By correlation analysis,we found that this set of data values of each case group with Hb MCV was no significant correlation between the values.3.By correlation analysis of this set of data, we found that in the group of Myelodysplatic syndrome ,Multiple myeloma,the RDW-CV value had negative correlation with Hb value.That means,with increasing degree of anemia,red blood cell width increases,respectively.4.In recent years,due to anemia caused by malnutrition significantly reduced,and a variety of reasons over the control diet,inadequate intake of nutrients that many of anemia caused by increasing year by year.
