Diuretic is the most importance and basic treatment,and it makes the basis of the treatments of chronic heart disease.The patients of chronic heart disease must reduce the heart preload as soon as possible.And then relieve symptoms.Now,besides the diuretic,we have another method to treat chronic heart disease,it is ultrafiltration. Kramer and other German doctors reported the continuous arteriovenous hemofiltration treatment to save a acute renal failure patient in 1977 firstly, which marks a new birth of blood purification therapy-a continuous renal replacement therapy expanding another important way and direction of the treatments ofheart failure.Compare with diuretic,ultrafiltration has many advantages,for example,it dosen't arouse diuretic resistance, electrolyte disturbance, lacking in strength, electrolyte disturbance lacking in strength...and activating the nerve endocrine system.etc。However, ultrafiltration.also has it's disadvantages,such as srousing low blood pressure, low blood potassium.short of breathe.So is there any way making the diuretic and ultrafiltration together better,doing a greater job in treating chronic heart disease.? Here, the writer designed this research,expecting to get a better effect and less disadvantageous function.The XinShuaiYaoFang(心衰要方)includes HuangQi 15gDangShen 12g ZeLanlOg FuPian 6g DanShen30-45g ZhiBanXialOg. Objective:The study aiming at the heart failure treatments.The hemofiltration process some deficiencies, in order to play to the advantage of hemofiltration in an even better fashion,improve patient's symptom and the quality of life.Hoping to achieve better clinical efficacy, I design the research.I design to treat the patients of heart failure with combined therapy of western, hemofiltration unite with Chinese medicine decoction.The decoction is called XinShuaiYaoFang(心衰要方) which is one of professor Ma XiaoChang's experiential Decoctions,,and it consists of.And the includes HuangQi 15gDangShen 12g ZeLan10g FuPian 6g DanShen30-45g Chuanjiaomu10g. How about the research and what's resule will it get?Methods:all the cases are from the paitents who was hospitalized in the cardiovascular ward and CCU of Chinese medicine academy of sciences-XiYuan hospital.totally I collect 47 patients and 44 patients in my research in the end.The research is a randomized control one,including 22cases in control group, experimental group of 22 patients in the control group. There are totally 4 times of treatments observing a month of treatment, body weight, serum potassium, BNP changes in incidence of hypotension, cardiac function, and symptoms improve and the ratio of patients being rehospitalized again in 2 weeks and 2 months.Results:1 The research finds that the difference of Two-Weeks-Rehospitalization between the test group and the pair group doesn't have statistical difference.But the Two-Month-Rehospitalization between them has significantly difference.2.On the BNP's impact:the two groups were significantly lower than befor,But the test group were not better than the control group, the BNP didn't have significant difference compared with the control group (P>0.05).3 On the incidence of hypokalemia:The test group which is treated with traditional Chinese medicine combined with blood ultrafiltration compared with the pure blood ultrafiltration P> 0.05,it means that there is no significant difference between them.4. On the incidence of hypotension:The test group was significantly lower than the control group,and their happen ratio were 41% and 73%.5.On body weight:The two groups both had a fine result after their treatments.But there was no significant difference between the them.6.On cardiac function:The test group improved more than the control group on the heart function, the difference was significant (P<0.05).7.The impact on the associated symptoms:The two groups both decreased in the impact of the associated symptoms,and the test group decreased more the later one (P<0.05); and respiratory difficulties, thirst heart palpitations,, fatigue night sweat.cold and other symptoms improvement effect significant.Conclusion:1.The Study finds that the difference of Two-Weeks-Rehospitalization between the test group and the pair group doesn't have statistical difference.But the Two-Month-Rehospitalization between them has significantly difference.The result may caused by the treatment of ultrafiltration and the slow-motion of Chinese medicine. XinShuaiYaoFang(心衰要方)can significantly improve cardiac function in patients of heart failure. It found that the test group can significantly expand coronary artery, to strengthen myocardial contraction, increase cardiac output, reduce myocardial oxygen consumption, improving the role of cardiac pump function, which can improve cardiac function.2.Both the test and the control group have decreased on the parameter BNP after treatments,but the test group which is treated with the Combining traditional Chinese and western medicine treatments didn't have a better result contrast to the control group.It may be related to the small sample size.3.The research didn't find the XinShuaiYaoFang (心衰要方) reducing more than the pure blood ultrafiltration group in patient's body weight. It means that the decoction doesn't have a obvious function on this aspects.4.The research found that the test group and control group both significantly reduced the incidence of hypotension, and the ratio of hypotension happened in the control group was 73%,and the test group was 41%,the difference between them has statistically significant5. On the aspects of improving symptom,the two group both got a fine result,improving significantly.And the test group did good in improving symptom,the difference between the two groups has statistically significant. It increase the patient adherence to receiving the treatment of hemofiltration.
