Objective: To evaluate the efficacy and safety of ultrafiltration used in congestive heart failure patients with retention of sodium and water. Method: Selected twenty heart failure hospitalization patients with retention of sodium and water (aged 74.5-82 years) who were failed after the medications(diuretic), 75% ischemic disease, ejection fraction 31-15%. Evaluating the condition of patients, ultrafiltration was initiated within 4.7-8.5h, velocity 25~50 ml/min, ultrafiltrate 0~600ml/h, until partes aequales liquid was filtered out. Reevaluation at discharged, involve the change of heart rate, blood pressure, bellows moist rale, hepatojugular reflux sign, edema and so on,pretherapy and post-treatment, detected rout blood,renal function, electrolyte and so on. Results: A total of 8,654-4,205 ml were removed with ultrafiltration initiated within 4.7-8.5h. after discharged, sixteen patients can sleep plat, the bellows moist rale, edema of lower extremity and paroxysmal nocturnal dyspnea were disappeared, the symptoms above were disappeared in three patients after discharged 24h, one patient only felt orthopnea improved. Pretherapy and post-treatment,the difference of weight has statistical significance (p<0.05), the blood pressure, renal function, and laboratory examination were unchanged(p>0.05).none death. Conclusion: In heart failure patients with retention of sodium and water, ultrafiltration is effectively and safely, It is decreases length of stay. |
