ObjectiveChina is a large agricultural country,and rural residents account for above90% of the population. Organic phosphorus is the most popular insecticide inour country,especially in the country,there are many person are poisonedbecause of touching,misusing,or commit suicide every year. The mainmechanism of its toxicity is when the organic phosphorus combines with theACHE; their product is phosphorylation CHE,it is difficult to decompoundacetylcholine gradually,acetylcholine will gather more and more,then we canfind the cholinergic nerve will be exciting firstly,and then is restrained. Thereare two kinds of clinical manifestation. One is muscarinic toxin-like symptom,that means the pasmodic smooth muscle and glandular secretion increasedwhich caused by the parasympathetic nervous become exciting. Another istobacco alkali-like symptoms,it is the expression caused by excessacetylcholine accumulated in the striated muscle neuromuscular junction. Toevaluate the content of organic phosphorus in the food and agricultural product is the important means to avoid organophosphorous insecticide poisoning.Meanwhile,to evaluate the content of organic phosphorus in patients withorganophosphorus pesticide poisoning is also one of the key points to clinicaltreat organophosphorous insecticide poisoning. The subject evaluates the effectof self-made portable detector for detecting organic phosphorus in the spottest,and the application effect to detect lavage fluid of the clinical patientswith organophosphorus pesticide poisoning.MethoMethod oneTo dilute the standard sample of methamidophos,omethoate,parathion,malathion,monocrotophos,acephate,phoxim,isocarbophosinto 5 teams with different consistency,choose 10 samples to be detectedseparately of every team,and set a blank controller of buffer solution inevery group,get 5ml of them to be detected. Detecting them by self-madeportable detector and the quick test card,then we can get the relevanceratio of different kind of pesticide and different consistency.Method twoDividing the 30 patients with organophosphorus pesticide poisoning orallyinto 3 type of mild,moderately,serious,deviding the lavage fluid into 3periods,detecting the content of ACHE in the blood of patient before they aretreated by ACA which is produced by HITACHI,then get 5ml of lavage sampleto detect the early stage,mid stage,final stage of lavage by self-madeportable detector and the quick test card,and set a blank controller group ofbuffer solution separately. Observe the relevance ratio of every type andevery period.ResultIt is confirmed that the detector can detect higher relevance ratio,it's 81.5%,and the effect of the detector is much better (P<0.01). There is obviousdifference in the relevance ratio among the early,mid,and final stage whendetecting the lavage fluid (P<0.05 or P<0.01); there is obvious difference in theratio of pesticide in lavage fluid in mid stage between moderately,serious andmild group (P<0.05 or P<0.01); there is obvious difference in the ratio in lavagefluid in final stage between moderately,serious and mild group (P<0.05 orP<0.01). The ratio of lavage fluid is the highest in the early stage,as stomachlavage is going on,the ratio becomes descend,and the poisoning degree ismore serious. ANOVA indicates that lavage fluid in different period has anobvious effect to relevance ratio (P<0.05),but different group has little effectto ratio (P>0.05),it confirmed that the detector is proportional and stabile indetecting AOPP lavage by the detector.ConclusioConclusionThe self-made portable detector is quicker and more exact than the test card,so the detector can be used effectively and widely for detecting organicphosphorus in the spot test. It has a positive effect to salve the poisoningpatients,it can be used to monitor the residual quantity of organic phosphorusin the lavage fluid,so we can know that if gastric juice is lavaged cleanly,ifit can attain the security standard,so the success rate can be improved. |
