| Objective:Based on years of clinical experience, professor Wei Wei summarized the pathogenesis of amblyopia which was inadequate endowment, Yuan Jing deficiency, and considered the treatment should be based on Bushen,Tianjin,Mingmu,Kaiqiao. He draw on the experience from professor Jiang Yuren who was a famous doctor which based on the theory of traditional chinese medicine treatment weak mentally, and made up the prescription "Yishi mixture"(hospital preparation) to treat amblyopia in children. The purpose of this study was to evaluate the efficacy of "Yishi mixture" objectively and scientifically and initially discussed mechanism, so as to guide clinical treatment of amblyopia in children.Methods:61 eyes of 36 patients based on inclusion criteriawere were enrolled in this study, divided into two groups, the patients in the control group received routine treatment includeing vision correction and training from fine work, the treatment group was treated with "Yishi mixture" based on routine treatment.4 weeks as a course, and 3 course in total. At the beginning of treatment and at the end of 4,8,12 weeks after the treatment were tested of vision, before treatment and after the end of 3 therapeutic courses were examined by pattern visual evoked potentia (P-VEP) and Optical coherence tomography (OCT). The improvement of vision, the peak latency times and amplitude of P-VEP, the neurosensory retinal thickness in the macula and the retinal nerve fiber layer(RNFL) thickness in around of optic disc were observed to evaluate the efficacy of "Yishi mixture" and initially discussed mechanism.Results:1. After 3 course of therapy, of the treatment group (34 eyes),18 eyes were mainly cured, the symptoms of 11 eyes were significantly improved, no effect in 5 case, the effective rate was 85.3%,the main cure rate was 52.9%; of the control group (27 eyes),8 eyes were mainly cured, the symptoms of 8 eyes were significantly improved, no effect in 11 case, the effective rate was 59.3%,the main cure rate was 29.6%. the treatment group was better than control group obviously(P<0.05), especially ametropic amblyopia and moderate amblyopia.2. OCT: After the therapy, the neurosensory retinal thickness in the macula and the retinal nerve fiber layer(RNFL) thickness had an decreasing tendency in different degrees, especially in the inferior inner macula, superior,nasal and inferior outer macula.3. P-VEP:After the therapy, P100 amplitude increased and latency prolonged slightly. However, only under the high spatial frequency stimulation the peak latency of P100 were significant changes. Conclusion:As adjuvant therapy, the "Yishi mixture" can significantly improve the cure rate of amblyopia in children, shorten the treatment time, and play a role in improving the form and function of Retina and visual cortex. |
