| Objective:Tumour is the most common disease of salivary glands, in the different anatomical locations of which the highest incidence is parotid tumors, accounting for approximately 80%. Benign tumor, one kind of parotid tumors, is the majority (75%),and malignant tumor, the minority (25%). In benign tumor, the most common is pleomorphic adenoma, though most of which is benign, it possesses certain aggressive growth characteristics, and scholars have made measureable researches to identify the factors affecting benign and malignant pleomorphic adenoma, thus being able to predict and intervene. In addition, people have different opinions on saliary gland benign, malignant pleomorphic adenoma and degree of malignancy. And it is difficult, just from cells form and tissue structure,to compare and prompt the more deep-seated reason involved in the pleomorphic adenoma, such as the growth characteristics of tumor cell, tumor cells division and proliferation, tumor suppressors etc. This paper, via detecting the expression of p73 protein and PCNA protein in benign or malignant pleomorphic adenoma, aims to find the law related with the benign or malignant pleomorphic adenoma, and to discuss the possible mechanism, thus to provide the theoretic and scientific basis for the identification, prophylaxis and treatment of benign and malignant parotid gland.Method:Collect 40 cases, aged from 45 to 72, of paraffin specimens of saliary gland pleomorphic diagnosed by oral surgery from 2007 to 2009,25 of which is benign and 15 is malignant; Another 20 cases of normal parotid tissue, near the benign pleomorphic adenoma, are chosen to be the control group. All the specimens are confirmed by pathology and without radiotherapy or chemotherapy before operation, and tested by immune organization S-P method. All data were statistically analysed by SPSS 17.0, The result take/p< 0.05 as the significant difference.Result:1. p73 is positive in expression in normal tissue and benign and malignant parotid gland pleomorphic adenoma, with a high rate being respectively as: 95%(19/20),96%(24/25),86.67%(13/15), and the difference is not statistically significant (p> 0.05). By comparing benign or malignant tumor group expression with normal parotid group expression respectively, the difference is not statistically significant (p>0.05). Comparison between the p73 protein expression of the two groups of benign and malignant pleomorphic adenoma shows the difference to be not statistically significant (p> 0.05).2. The positive expression rate of PCNA protein expression in parotid normal tissue is 30% (6/20), and 72% in benign pleomorphic adenoma (18/25), and 86.7% in malignant pleomorphic adenoma tissue (13/15). The positive expression rate of PCNA protein expression in benign and malignant pleomorphic adenoma tissue is significantly higher than that of normal tissues (p<0.05), with a statistical significance. The expression of benign or malignant tumor group is respectively compared with normal parotid group and the difference is statistically significant (p<0.05). Comparison of PCNA protein expression between the 2 groups of benign and malignant pleomorphic adenoma shows to be statistically significant (p<0.05).Conclusion:1. The expression of p73 protein maybe nothing with the occurrence and development of benign and malignant pleomorphic adenoma.2. The excessive expression of PCNA protein maybe related with the occurrence and development of benign and malignant pleomorphic adenoma.3. PCNA can reflect the proliferation of the parotid tumors, thus to be used as one of the referenced markers of the possibility and grade malignancy of parotid gland neoplasms, with an excessive high expression to be considered as evil change tendency. The detection of PCNA will help to recognize the essence of precancerous condition and to guide clinical treatment, and have great reference values in the parotid gland pleomorphic adenoma diagnosis and prognosis.4. The high expression of PCNA in malignant pleomorphic adenoma points out that its proliferation activity is higher than that of benign pleomorphic adenoma and normal parotid tissue, and the proliferation activity of benign and malignant pleomorphic adenoma is significantly higher than that of normal parotid tissue. Thus proved the important significance of PCNA proliferation detection in judging malignant hyperplasia pleomorphic adenoma. With the advantages of histological and cell proliferation analysis, PCNA, not only can provide support for the diagnosis of malignant pleomorphic adenoma, but also can provide basis for clinical treatment and prognosis assessment. |
