Objective To explore the clinical efficacy of posterior operation of degenerative lumbar scoliosis with stenosis.Methods The clinical data of 42 cases with degenerative lumbar scoliosis with stenosis treated via posterior approach in our department from June in 2003 to October in 2008 was analyzed retrospectively. There were 16 males, 26 females, with the mean age of 66 years (ranged from 47 to 74 years). The disease history were 9 months to 10 years, with the mean 3 years. There were group A and group B, 20 cases(Group A) underwent posterior laminectomy alone. 22 cases(Group B) underwent posterior decompression , bone graft and internal fixation . The Cobb's angle and lordosis angle of lumbar of all patients were analyzed preoperatively and postoperatively, respectively. The preoperative and postoperative function of the patients were evaluated by Japanese Orthopedic Association(JOA) scoring system and measured the recovery rate.Results All the cases were followed up from 6 months to 54 months, averaging 24 months. The postoperative Cobb's angles of Group A and Group B were 14.00±4.140 and 31.0o±8.34o, the lordosis angles were -36.0±-3.34o and-33o±-5.47o , and the clinical outcomes evaluated by Japanese Othopedic Association(JOA) were 17.5±2.80 and 24.8±4.12 ,respectively. To measure the recovery rate of Group A and Group B . The data were analyzed statistically by the SPSS13.0, the differences between Group A and Group B was statistically significant (P<0.05).Conclusion Posterior decompression ,bone graft and fusion , internal fixation can attain more satisfied clinical effect than Posterior laminectomy alone in the treatment of degenerative lumbar scoliosis with stenosis; the former can reconstruct the alignment and recover the stability of spine, which is important to keep up surgical efficacy.
