Object This study determined hTERT(human telomerase reverse transcriptase) in keloid,in order to evaluate the role of the telomerase activity in the formation and development of keloid.Methods There is 4 groups in this study:A group is normal skin samples,12 samples aquired from plastic surgery;B group is keloid samples,16 samples of keloids proved by clinical diagnosis;C group is improved keloid samples after treated by Triamcinolone Acetonide,10 samples;D group is mature scar samples,15 samples.All the samples were made into paraffin blocks after fixed by 10%neutral formalin.Protein expressions of fibroblasts hTERT in sections of four groups were inspected with immunohisto-chemical method.Observe the positive rate of four groups,and test the results by chi-square criterion with SPSS11.5 statistics software.Results The expression of hTERT in keloids(62.5%) is higher than improved keloids after treated by Triamcinolone Acetonide(10%,P<0.01),mature scars(0,P<0.01) and normal skins(0,P<0.01),That in the improved keloids after treated by Triamcinolone Acetonide has a sign to be increased but no stastistical difference with other 2 groups;And the expression of hTERT in mature scars is found to be the same with that in normal skins.Conclusion①The activation of telomerase plays an important role in the emergence,development and prognosis of keloid.②The emergence of keloid is the result of mutiple procedures and stages,induced by multi-genes and comprehensive factors.The activation of telomerase may occur in the later phase,which may be one of common passageways by which the comprehensive factors play roles in the emergence of keloid.③Fibroblasts comprismultiple subpopulations,and some subpopulations may acquired lesions or mutations of multiple genes.During the process of proliferation and division induced by trauma,the control of proliferation and apoptosis is in loss of balance,which leads to an abnormal increasing of telomerase activation,and finally keloid emerges.④After the effective treatment,the fibroblasts that are still hTERT-positive may the lead to the tecurrence of keloid.
