Background and objectiveNowadays, people pay more and more attention to the ischemic cerebrovascular disease due to its high incidence rate, high mutilation rate and high recurrence rate. This paper includes two chapters. The first chapter is to analyze the characteristics of distribution of cerebral artery stenosis in patients with ischemic cerebrovascular disease of Qingdao and the influence of age.The second chapter is to explore the prognosis factors of patents with symptomatic intracranial arterial stenosis.MethodsChapter 1:Totally 306 patients with ischemic cerebrovascular disease confirmed by the digital subtraction angiography had cerebral artery stenosis. They were categorized the young-aged group,middle-aged group and old-aged group. The characteristics of distribution and the level of stenosis of cerebral artery stenosis were analyzed.Chapter 2:Totally 86 patients with symptomatic intracranial arterial stenosis confirmed by the digital subtraction angiography were followed up for 1 year. The major outcome of end-point was stroke events. The characteristics of distribution of intracranial arterial stenosis were analyzed. The multiple Logistic regression analysis was used to evaluate the association between the risk factors and the outcome.ResultsChapter 1:As for incidence of intracranial and extracranial arteriostenosis,128 of 306 patients were of intracranial stenosis only and 78 extracranial stenosis only.As for incidence of anterior and posterior circulation arteriostenosis,142 were anterior circulation arteriostenosis only and 63 posterior circulation arteriostenosis only.As for incidence of single or dual stenosis,96 were of single stenosis and 21 dual stenosis. The incidence of single stenosis of the old-aged group(23.28%) was lower than that of the middle-aged group(17.87%) and the young-aged group(7.36%)(P<0.01).The incidence of intracranial arteriostenosis of the young-aged group(83.62%) was higher than that of the middle-aged group(44.79%) and the old-aged group(28.30%)(P<0.01).The incidence of the anterior circulation arteriostenosis of the old-aged group(31.45%) was lower than that of the young-aged group(74.51%) and the middle-aged group(56.25%) (P<0.01).Chapter 2:Of the 86 patients,80 completed the 1-year follow-up period. The rate of the end-point occurrence was 33.75%.The rate of stroke recurrence was 25%.Age≥60 years old(OR 3:60;95%CI 1.23-24.86;P =0.03),females(OR 4.05;95%CI 1.21-21.12; P =0.03),NIHSS score>1(OR 2.40;95%CI 1.36-38.32;P =0.02) and multiple intracranial arterial stenosis(OR 3.89;95%CI 1.14-13.93;P=0.03) were associated with the outcome.Conclusions1.The incidence of stenosis of intracranial artery was higher than that of extracranial artery in 306 patients with cerebral artery stenosis of Qingdao.2.The distribution of cerebral artery stenosis varied with age.3.Age,gender,NIHSS score and the number ofintracranial arterial stenosis of patents with symptomatic intracranial arterial stenosis may be the predictors of stroke events in one year. |
