Diabetic nephropathy is one complication on kidney from Diabetes mellitus,which major symptom is continuity proteinuria on diabetic.In order to get TCM pathogenesis of diabetic nephropathy and the regularity of different stages on TCM pathogenesis with the gender,age and laboratory indices,this work is meaningfully to open out the character of the TCM syndrome.This research is part content of the fifteen national science and technology tackle keyproblem-the general project research of the whole range diabetic nephropathy prevention and cure.The research proceeds the questionary of diabetic nephropathy established according to task group,collecting the Diabetic Nephropathy sufferer came from Dozhimen Hospital of Peking Chinese Medicine University etc.All the patients are chosen base to the same standard of general physical condition,four diagnostic methods information of TCM and correlative laboratory indices.We choose the case with complete data to proceed statistics analysis,using SPSS counting software to run the x~2 checking and factor analysis method in order to sum up the distributing of TCM syndrome and summarize the feature and developing principle of TCM pathogenesis in the whole stage of diabetic nephropathy.We discovers that the sufferer are mostly belong to deficiency of Qi and Yin,deficiency of Qi,Yin and Yang,deficiency of Qi,blood and Yang in respective third,fourth and fifth period of diabetic nephropathy.Also phlegm and dampness,stagnation of blood stasis penetrate in all the periods of Diabetic Nephropathy,especially the dampness and turbid are becoming more and more obvious with the developing of the disease.We find that the deficiency mixed with excess syndrome are very common in all periods of Diabetic Nephropathy,surpassing the ratio of 84%and upgrading to 92.62%in third to fifth periods.Many patients in third,fourth period of diabetic nephropathy have more than three syndromes combination,and accordingly have five syndromes combination in fifth period.The results of factor analysis correspond with the 7 groups of TCM syndrome in questionary,and prove that deficiency of Qi, deficiency of Xue,deficiency of Yin,deficiency of Yang,phlegm and dampness,dampness and turbid,stagnation of blood stasis are foundmental TCM syndrome element in diabetic nephropathy.According to results of the clinical research,we get following conclusions:1. The results of factor analysis confirm some well-known experts' clinical experience,and hint us that according to the clinical Epidemiology principle methods,it is capable to carry out the strictly designed clinical research base to the golden standard of using expert experience as comparison.2.The main TCM syndrome of sufferer in compensation period of diabetic nephropathy renalfailure are deficiency of Qi,Yin and Yang,and in decompensation period are deficiency of Yang.And the syndrome of phlegm and dampness,stagnation of blood stasis run through whole developing process,coinciding with the traditional TCM pathogenesis.In short the pathogenesis of this disease is complicated,such as deficiency syndrome accompanied with excess syndrome.The results prove that professorlv'idea of differentiation of symptoms and signs on deficient syndrome and sthenia syndrome,and treatment of activating blood circulation to dissipate blood stasis and soften hard lumps and dispel nodes is scientific.
