Immortals Decoction Prevention Aromatase Inhibitors In Breast Cancer Patients With Bone Loss Caused By Clinical Observation

This essay consists of two parts:literature review and clinical research.Literature review:This part retrospectively analyzed the fundamental and clinical research advances on bone-loss associated with breast cancer at home and abroad,then discussed about the mechanism,diagnostic criteria,clinical manifestation,advances in management of bone loss in breast cancer patients with western medicine.Overall,the cancer treatment induced bone-loss(CTIBL) especially aromatase inhibitor-associated bone loss(AIBL) always leads to obvious joint syndrome,even osteoporosis and fracture.All these adverse effects worsened the patients' life quality which may resulted in the cease or change of the treat arrangement.This issue has been paid extensively international attention,but there is few reports on it in Chinese medicine field.Clinical research:Objective:TO investigate the clinical efficacy and safety of Er Xian Tang modified as is used in the prevention and treatment on bone loss of breast cancer patients using AI.Method:Choose breast cancer patients in menopausal who are taking AI and then divide them into two groups randomly:combination treatment group and pure western medicine therapy control group.According to ASCO Breast Cancer Bone Health Guidelines:all the patients were given Caltrate With Vitamin D Junior Tablets 1200mg po qd,and Alendronate Sodium 70mg qw,according to patients' BMD T-score and risk factors.The patients in treatment group were given additionally decoction of Er Xian Tang modified po,twice a day.Course of each group at least lasts for six months.Record the incidence of fracture,clinical symptoms,bone mineral density(BMD),estrogen(E₂),and other cancer indexes before and after the therapy. Result:27 patients were studied,and 15 in treatment group,12 in control group. The effective rate of finger pain remission in treatment group is 66.7%,in control group is 8.3%(P=0.005);The effective rate of lower back and knee pain remission in treatment group is 53.3%,in control group is 8.3%(P=0.019).The treatment group is better than the control group in relief of the symptoms.There is statistic significance between two groups.The treatment group is better than the control group in improving the BMD of L1-L4.There is statistic significance between two groups(P＜0.05).Followed survey was carried till April 2009,every patient in both groups has no fracture,and breast cancer is stable.Conclusion:Er Xian Tang modified is effective to bone loss of the breast cancer patients using aromatase inhibitor and it could maintain the BMD,atso give prevention from osteoporosis and fracture.Er Xian Tang modified is proved to be able to relieve the joint symptoms,and improve life quality.There needs a more-sample, multi-center,longer-follow up survey study to provide more evidence.
