| Objective:Rheumatoid arthritis(RA)was a kind of autoimmune disease which mainly appeared as symmetrical poly-arthritis of four limbs and which was characterized by chronic inflammation of synovium of joint and progressive destruction of joint. Its main clinical manifestation was joint pain,early disability and shorter duration of life.It increased the burden of both individual and the society because of its continuous distress and Long-term of treatment.The severity of RA fluctuated during its chronic course.The crest-time of RA activity increased the pain of the patient and accelerated the speed of joint destruction.Judgment of RA active stage (2002,ACR):①more than 6 swelling joints;②more than 6 painful joints;③early morning stiffness last more than 45 minutes;④ESR is more than 28mm/h;⑤CRP is no less than 1.5 times of normal value.Item①and②are essential, and 2 items of the item③,④,⑤are necessary.At present,modern medicine was lack of the method of radical cure of RA.The utmost control of inflammation and abatement of bone erosion was its curative aim But the traditional Chinese medicine had its distinct cognition and affluent experience in the cure of RA,which had significant effect in reliefing the pathogenetic condition,delaying the advancement of the state of an illness,and improving the patient's life quality.The rheumatology and immunology department of Guang'anmen hospital of China academy of Chinese medical science discovered during the long-term diagnosis and cure of RA that the syndrome of most active RA was the syndrome of humid heat complicated with stagnant blood,which could be well cured by the method of clearing heat and promoting diuresis and activating blood circulation to dissipate blood stasis.On these grounds,we proposed that the key pathogenesis of active RA was damp heat blocking the joints and blood stasis obstructing the collaterals,and the key method was clearing heat and activating blood circulation.We prepared the composition misture of clearing heat and activating blood circulation to use in the animal experimental research.TypeⅡcollagen induced arthritis was the most frequently used and relative ideal arthritis model in today's experimental research into RA.And the pathological research of it was very proficient.TypeⅡcollagen induced arthritis could relatively well reproduce the course of RA,and relatively objectively reveal the effect of the curing method on RA.We carried out this research topic to tentatively approach the mechanism of the effect of the method of clearing heat and activating blood circulation on RA.Method:Used 40 Wistar rats,randomly separated into normal control group,model group, large dose group and small dose group(every group had half male and half female rats).Established typeⅡcollagen induced arthritis model.After 2 weeks,the CIA rats were fed with the composition misture of clearing heat and activating blood circulation everyday for 24 days.The volume of postpedes of the rats were recorded.The pathological changes of arthral section with HE stain were observed under light microscope.Results:1.CIA model was successfully established.2.The joint swelling could be relieved by the composition misture of clearing heat and activating blood circulation.The effect of large dose was better than that of small dose.3.Under light microscope, the composition misture of clearing heat and activating blood circulation could improve the pathologic state of CIA model.And the effect of large dose was better than that of small dose.Conclusion:1.The method of clearing heat and activating blood circulation had therapeutic action to CIA.2.In this initiatory research,we found the mechanism of the method of clearing heat and activating blood circulation was:this method could repress the inflammatory cell infiltrate into the synovial tissue of joint,and repress the blood vessel hyperplasia,and stop the formation of pannus.So the method of clearing heat and activating blood circulation could lessen the destruction of articular cartilage and subcartilaginous bone,and could improve the pathologic state of the joints.3.The deeper molecular mechanism of the method of clearing heat and activating blood circulation was waiting for more penetrating research. |
