Objective:Cervical spondylosis of vertebral artery type (CSA) is a syndromeinvolving dizzy which are caused by vertebrobasilar artery insufficiency.CSA is a clinical common and frequently-occuring disease. It tends toreoccur and interferes with patient's work and the life severely.Jiaji points are extra meridian points also called Huatuo Jiajipoints or Tuoji points. They are often used in clinic because of theirwide therapeutic scope and significant as well as safe effect fromancient times till now. The clinical reports about their application onCSA are increasing in recent years. However, little is known on the studyof their analgesic time and mechanism. So, the study is to observe theanalgesic and clinical effect as well as influence on the levels ofEndothelin (ET) and calcitonin generelated peptide (CGRP), aiming toinstruct clinic and reveal the mechanism of the therapy of jiaji pointinjection for patients with CSA.Methods:60 cases of CSA came from inpatient and outpatient department of NO1hospital inWuhan. The standard of diagnosis are separately based on thestandard made by the second cervical spondylosis conference in 1992 andnational TCM bureau in 1994. All the patients are diagnosed by X-ray orCT and TCD and clinical syndrome. 60 cases of CSA were randomly dividedinto jiaji point injection group (30 cases) and electro-acupuncture group(30 cases). Select the C5 and C6 Jiaji points of double side half inchfrom the middle corresponding to the protrusion under the upper spinouspeek. Select the C5 and C6 Jiaji points of double side half inch fromthe middle corresponding to the protrusion under the upper spinous peekConnect HANS after acupuncture. The stimular parameter is wave ofmodulated frequency. The width of wave is 0.6 second when 2Hz, 0.4 second when 15Hz, 0.2 second when 100Hz. The genernal electric tensity isranging from 10 to 20mA which can be accepted by patients. Thirtyminutes therapy every day. Ten days a treatment course. The level of ETand CGRP were measured by radio-immunity methods two times. One is beforethe first treatment, another is after one treatment course. To explorethe changes and significance of endothelin (ET) and calcitoningenerelated peptide (CGRP) in cervical spondylosis of vertebral arterytype (CSA).Results:1. Curative effect: The total effective rate was 96.7% in the jiaji pointinjection group and 93.3% in the electro-acupuncture group. Thecurative effect has no difference between two groups.2. Before treating, the level of ET in two groups were higher than thosein normal reference range. The level of CGRP in two groups were lowerthan those in normal reference range. After treating, the level of ETwere lower and the level of CGRP were higher which have obviouslydifference than pre-treating (P<0.01).Conclusion:1. Both C5,C6 Jiaji point injection and electro-acupuncture can treatCSA;2. C5 Jiaji point injection can decrease the level of ET and increase thelevel of CGRP, and can better improve the condition of vertebrobasilarblood flow than electro-acupuncture group.3. ET and CGRP may play an important role in regulation of nerve and fluidfunction of CSA.
