Part one BackgroundThis thesis systematically expatiates on the condition of dysmenorrhea treated by Chinese medicine from the aspects of the causes and pathogenesis, syndrome differentiation, therapeutic principles and methods. Also presents the pathogeny of primary dysmenorrhea and the therapeutic methods of Western medicine. After making a comprehensive view over the therapeutic actuality of traditional Chinese medicine, points out the insufficiencies and discusses how to do better in the future.Part two Clinic investigationProfessor Mei Qianyin indicates that primary dysmenorrhea is mainly because of qi stagnation and blood stasis. She treats it with Dingtong Decoction which has the function of regulating qi flow and blood circulation, eliminating blood stasis to relieve pain. ObjectiveTo evaluate the curative effect of Dingtong Decoction on primary dysmenorrhea (qi stagnation and blood stasis) and its safety. And to provide basis for Dingtong Decoction's application in clinic and further scientific researches. MethodsSixty-six cases of primary dysmenorrhea (qi stagnation and blood stasis) were chosen. They were divided randomly into Dingtong Decoction treatment group(n=36) and Yuanhu Zhitong Tablet control group(n=30). The distribution of age, course of disease, the patient's state was comparable. And the integral of dysmenorrhea, main symptoms of the two groups had no statistical difference ( P > 0.05 ) .We graded according to the standard of perfection.The patients in the treatment group were given Dingtong Decoction(Radix Angelicae, Radix Paeoniae Alba, Rhizoma Chuanxiong, Fructus Lycii, Fructus Toosendan, Rhizoma Corydalis, Olibanum, Myrrha, Rhizoma Cyperi, Radix Linderae) twice a day,while in the control group were given Yuanhu Zhitong Tablet, five tablets each time and three times a day. Both started at five days before menstruation, and lasted seven days. The period of treatment contained three menstrual cycles.We observed the degree and duration of pain, the quantity, color and texture of menses and other symptom's improvement after the treatment. The blood routine, urinary routine, PT, TT, APTT, hepatic function and renal function of the treatment group were examed before and after the treatment.Then evaluated the curative effect according to the determinant criterion.The measuement data had been analysed by t test, and the enumeration data been analysed by chi-square test. While the ranked data had been tested by Ridit analysis. Results1 The cure + markedly effective rate of the treatment group is 77.8%, and the total effective rates is 91.7%. While the cure + markedly effective rate of the control group is 53.3%, and the total effective rates is 73.3%. There is obvious statistical differencebetween the two groups ( P < 0.05 ) . It states that the clinical effect of the treatment group is better than that of the control group.2 Both can significantly depress the integral of dysmenorrhea after the treatment ( P < 0.01) .Comparison between the two groups shows evident difference ( P < 0.05 ) .3 The total effective rates of TCM symptoms are 94.4% and 76.7%, while the cure + markedly effective rate are 75% and 50%. We can find out the curative effect of the treatment group is much better than that of the control group(P < 0.05).4 Both can obviously alleviate pain, ameliorate the correlate symptoms such as scanty menstruation, impeded discharge of menses, purplish and dark color menses with blood clot, alleviation after the removal of blood clot, distending pain in the breasts before menstruation, purplish and dark tongue or with ecchymosis, wiry pulse ( P < 0.01). There is significant difference between the two groups on the same symptoms(P < 0.05).5 Following up the cured patients of the treatment group, 4 out of 15 recrudesced. While in the control group, 2 out of 7 recrudesced. There's no statistical differences between the two ( P > 0.05 ) .6 After the correlativity analysis, we finds out that the curative effect is correlated with the patient's state.7 There is no abnormal changes for the...
