1. PURPOSEThis study is aim to introduce the cooperative learning method to the area of chronic disease management of community health service(CHS).Through this research we can explore the effect on improving the quality of education and enhancing the work ability of community staff and improving the health of community residents. Sum up the feasibility, the application of the conditions and the work skills of the implementation in China; build a practical model of cooperative learning in chronic disease management. Finally try to provide some references for other regions and domain to use this method.2. METHODS2.1Literature searches: through out the literature research and network to get correlative materials, link the characteristics of the management of chronic disease in community and the foreign cooperative learning experience in health area, build the theoretical model of cooperative learning in the chronic disease management.2.2 Case-analysis: analyze the type institutions and then collect the qualitative and quantitative data to analyze the feasibility and effect.2.3 Stakeholder: Select different perspective and representative indicators to analyze the effect of cooperative learning to each interest group. Summarize the operability, and then try to build a practical model of cooperative learning in chronic disease management.3. RESULTS3.1 Have changed the working of the staff: changed waiting to initiative, established a general practitioner team to work regularly; Have extended the work area: improved the type and quantity of service; Have increased the workload of staff: improved the quantity and the quality of daily work both; Have changed the attitude and behaviors in their work: from passive to active, from reluctant to willing to serve.3.2 Have improved the ability and the quality and the level of chronic disease management of CHS; Have promoted the healthy competition among institutions and enhanced the overall level of chronic disease management of the region behind; Have narrowed the city's gap between the management of chronic diseases and provided a systematic and scientific thinking for the realization of large regional management of chronic diseases.3.3 The ability changes of staff have brought the increase of compliance and satisfaction, and then have slowly changed their attitude and behaviors about chronic disease.3.4 Because of the short duration of the implementation of the project, the rate of the chronic disease management of coverage, control, abnormal were increased, but not obvious.4. CONCLUTIONS4.1 The effect of the practice of cooperative learning: As a means, it has made the externalization of tacit knowledge and the share of experiences come true and accumulated and summarized variety of chronic disease prevention and management experiences; As a mechanism, it ensured that the staff's ability could be improved based on the learn and share from each other; As a driving force, it changed the staff's attitude and behavior about the chronic diseases prevention and control; As a link between the close of the staff or the doctors and patients, it helped to build up a spirit of cooperation and improved the ability of chronic disease management; As a platform, it provided the opportunity that the theory was applied to practice and the developing and reactionary space; As a model, it supplied and corrected the deficiency of community education.4.2 It is feasible to introduce this method to chronic disease management, but needs conditions: The leadership side- attach importance to this area and funding support; The agencies supply side- the right work ethic, experience knowledge, appropriate incentive mechanism; Community demand side - a stronger awareness of health; Social side - external resources to support and understanding of differences in cultural backgrounds.4.3 The work skills of the cooperative learning implementation: To balance the development of different institutions, as well as the region's economic level; Define the participant roles and responsibilities; To build different levels of Learning Community; To establish knowledge-sharing support system; To perfect chronic disease management incentive mechanism; To found reasonable and effective evaluation; To prevent and avoid risks; To do a good job in quality control process.4.4 Based on the practice of cooperative practice activities, we focus on the analysis of typical institutions in the chronic disease management practice, and then build practical model of cooperative learning methods in chronic disease management.5. SUGGESTIONS5.1 Aim at the cooperative learning itself: To expand the scope of training; To improve the training quality; To extend the duration of the project; To ensure effect of implementation; To strengthen the experiences; To promote the technical guidance; To change the form of exchange; To construct the innovative platform.5.2 Aim at the introduction of cooperative learning to the area of the chronic disease management: To seek great attention of higher department as well as the support of external resources; To adjust staffing; To innovate the working; To improve the incentive and restrictive mechanism; To perfect the performance evaluation system; To create the cooperative mechanism of competition between institutions.
