Objective:This is the study to compare the efficacy between mitral valve (MV) repair and MV replacement surgery. The main purpose of this study is to clarify the morbidity and prognosis of different surgical technique on MV disease. Moreover, we would like to understand the future perspectives of surgical approach on MV disease at the meanwhile.Methods:The data was collected from First Affiliated Hospital of Guangxi Medical University of Cardiac Surgical Unit from September 2013 to October 2015, a total of 895 patients who received mitral valve surgery, we randomly selected 61 cases, of which exclude the cases doing aortic valve replacement or CABG (coronary artery bypass grafting) at the same time and set up the database. Then the patients were divided into MV repair and MV replacement groups, carrying out the retrospective analysis and review. Analysis of preoperative cardiac function, intraoperative surgical data, preoperative atrial fibrillation, preoperative left atrial diameter, left ventricular end systolic diameter, left ventricular end diastolic diameter, preoperative cardiothoracic ratio, postoperative left atrial diameter, left ventricular end systolic diameter, left ventricular end diastolic diameter, left ventricular ejection fraction changes and postoperative cardiothoracic ratio.Results:We found that no significant differences in left ventricular function or survival at 6 months after receiving repair or replacement surgery. Both of the groups were significantly improved in heart function after the surgery. Left atrial diameter, left ventricular end systolic diameter, left ventricular end diastolic diameter of repair group were significantly improved (P<0.05); left ventricular ejection fraction also significantly improved(p<0.05). Left atrial diameter, left ventricular end systolic diameter, left ventricular end diastolic diameter, left ventricular ejection fraction of replacement groups were significantly improved(p value<0.05). CTR in both repair and replacement groups were significantly improved results compare with the data before surgery. Both groups at operation periods, aortic cutting period and extracorporeal circulation period were not statistically significantly (p value>0.05). Age and sex parameters of both groups were not statistically significant.Conclusion:l.Both MV repair or MV replacement surgery are effective methods that can improve heart function.2. Patients from repaired group lost less amount of blood compared to the replacement group.3. Compared with the replacement group, patients from repair group show shorter hospital stay.4. MV repair is not only less damage to the heart but also postoperative complications. Better recovery of cardiac function during follow up and also quality of life.
