Investigation On States Of Community-based Rehabilitation Of Stroke Patients In Changchun City

Stroke is one of the most important diseases that lead to death. 3/4 survival patients of stroke patients about lose their ability to work. A heavy burden is given for increasing disability for the patients'family and society in the stroke. It has been a common recognize and an important role to community-based rehabilitation for stroke. Community-based rehabilitation can help functional recovery of stroke patients, improve the stroke patient's mental status, and improves quality of life and social function of stroke patients. More than a dozen cities and districts get the model demonstration counties of the disabled'community rehabilitation in Jinlin province, but it has not get a detailed analysis to community-based rehabilitation of stroke patients.MethodsReferences searching, questionnaire survey and site investigation were used to investigate resources and equipment of community-based rehabilitation, service resources, and the rehabilitation of stroke patients in the community respectively.The rehabilitation of basic situation of Changchun community health service centers (stations), the cognitive of technical personnel and 50 community-based rehabilitation's physician were investigated. 235 stroke patients in five committee with stratified random sampling method were investigated to understand the community rehabilitation of stroke patients in Changchun city status and rehabilitation needs, explore new ideas for the development of community-based rehabilitation, provide theoretical basis to improve community health services.All the data were input to form the database with Excel 2003 and spss13.0 software.ResultsThere are 34 Community Health Centers in five old districts of Changchun City. There are 50 health professionals in the first training in the work of community-based rehabilitation, 62% is middle title and senior title. 52% is bachelor degree. The community-based rehabilitation of stroke was supported by most of them, but not carried out completely. The reasons included unskilled community-based rehabilitation, short training time, unbelievable residents, imperfect referral system and so on. The community-based rehabilitation should be carried out by further leadership attention, workmate cooperation, awareness of rehabilitation, improvement of the referral system, superior technical support hospital, increased publicity, and the whole society's understanding of community rehabilitation.235 stroke patients were investigated include 194 males (82.5 percentage of the total number) and 41 female (17.4 percentage of the total number). Most of the patients is 65-75 years old (52.7 percentage of the total number). Received high school education among them accounted for 55.7%. Workers accounted for 52.3%. Most patients'income is 1000-2000 (43.4 percentage of the total number).Disability rate is 88.0% in 207 investigated patients who remained a variety of obstacles. The patients had low recognition rate for community-based rehabilitation, while 54.4% patients didn't understand or know. The patients had a high demand for rehabilitation (65.5 percentage of the total number), especially in low-income group. Few early stroke patients got good community-based rehabilitation, only 5.5%. The most factors for low utilization of community-based rehabilitation was low income (43.2 percentage of the total number). The other were that the patient didn't understand the knowledge about the community-based rehabilitation.Stroke patients got the knowledge of the community-based rehabilitation from the doctor, books and mass media. Different community-based rehabilitation ways were chosen by different ages, different income. The patients had the intention to carry community-based rehabilitation smoothly. First is ensuring the efficacy. Second is reducing costs. The Rehabilitation staff's attitude for patients and referral facilities were necessary.ConclusionsThe facilities of community-based rehabilitation are quite good in Changchun city, while the work has not launched completely. Community rehabilitation physicians are serious shortage, and their professional skills should be improved. Moreover, effective referral system didn't establish yet. The rehabilitation of stroke is lack. Although the stroke patients have high requirement, but little knowledge about community-based rehabilitation is know. Economic factors could affect the enjoyment of rehabilitation services. The knowledge of community-based rehabilitation is not propagated widely, and not be understood by stroke patients.SuggestionRehabilitation physicians should strengthen their own professional levels. Information communication between large hospitals, professional rehabilitation and community health center should be strengthened Regulations and policies of community-based rehabilitation services should be completed. The importance of universal community-based rehabilitation should be universalized in the whole society.
