objective:To be clear, B-type natriuretic peptide (BNP) can better reflect the heart function.but local left ventricular function and B-type natriuretic peptide (BNP) is relevant, it is not clear. This study is to investigate the correlation of BNP level and the local left ventricular function in patients with coronary heart disease.Methods:1.In early moring gathering experiment group and the control group fasting circumference blood serum.Measure the serum BNP levels by RIA of all cases.2. First selecting two-dimensional image acquisition of all people (73 patients with coronary heart disease and control group,18 patients) by American Phillips iE digital color Doppler ultrasonography of S5-1,L11-3 heart probe.Analysising Three-dimensional images by processing system 4D-LV Analysis software which comes from Germany.Left ventrical is divided into 16 segments according to the United States Echocardiography Association.Automatically Obtaining 16 segments of end-diastolic volume(EDVR),end-systolic volume(ESVR),partial stroke volume(SVR),regional ejection fraction(EFR),And calculating segment-overall ejecton fraction(EFRG).Secondly,Through transthoracic echocardiography,M-type ultrasound under the left ventricular long axis detecting the case group and control group interventricular septum thickness(IVST),left ventricular end diastolic diameter(LVDd),left ventricular end systolic diameter(LVDs),Simpson measurement of mitral early diastolic peak velocity Mve,atrial systolic peak velocity Mva. Calculating E/A ratio.3.Statistical analysis:All data are indicated by mean±standard deviation and analyzed by SPSS13.0.Statistical methods between groups use T test,linear correlation is used between the different indicators.P<0.05 considered significant.Results:Coronary heart disease patients of all 16 segments,there are 7 segments of EDVR higher than the corresponding segments of the control group,and 15 segments of ESVR is too.But there are 14 segments of SVR,EFRG lower than the control group,and 15 segments of the local ejection fraction (EFR) lower than the corresponding segments of the control group too.BNP in CAD patients is more higher than normal group,and was a negative correlation with left ventricular ejection traction(LVEF)(r=-0.72,P<0.01),linear regression equation was y=1267.53-0.72x.Left anterior and lateral walls disease's patients with higher BNP level than with the right ventricle's.The difference was significant. BNP level is different in patients with different diseased region,and is negatively correlated with EF (r=-0.78,P<0.01), linear regression equation was y=839.19-0.78x.Conclusion:1 BNP production mainly comes from the left ventricle,and the levels can reflect the overall situation.2 Different diseased region of CAD patients have different sizes contribution to the whole ejection function.Left ventricular anterior,lateral,anterior septum have the greatest.3 BNP level is more higher in left ventricular anterior,lateral,anterior septal lesions than others.Indicating that the changes of BNP has a high degree of agreement with local cardiac function. BNP can reflect the regional cardiac function states.
