Clinical Research On Nontraumatic Osteonecrosis Of The Fermoral Head With Interventional Therapy

Objective:To observe the rencent effection of Nontraumatic Osteonecrosis of the Fermoral Head with Interventional Therapy.To discuss the relationship between the changes of local blood vessel and Adult Avascular Necrosis of Fermoral Head with Digital Subtraction Angiography (DSA)Methods:A total of thirty-five patients(46hips) with adult avascular necrosis of the femoral head according to the standard of diagnosis and selection received the interventional therapy in department of orthopedics,Zhangzhou hospital of TCM, Fujian province from April 2006-June 2009 were selected. The medicines were injected into the hips by using Seldinger's technique. The cilinical efficacy and imaging were analyzed by the inflution on Self before-after treatment. To observe the changes of local blood vessel after Interventional Therapy with (DSA), we studied and researched the change of local blood supply after Interventional Therapy for Adult Avascular Necrosis of Fermoral Head.Results:Thirty-five patients(46hips) were all descriped in DSA:1. Describing index:Before the first interventional therapy,the number of femoral head's blood vessel wasslender and the number of ramus anastomoticus was rare.The blood vessel was stegnotic and even was interrupted.The stained range was localized and dark.After the interventional therapy,the embolic blood vessel became easy and smooth.The blood vessel was thick and excurrent.The stained range was wide and light-coloured.2.Numerical index:â‘ To compare the blood vessel's caliber of preoperative and postoperative, there was obvious significant difference between preoperative and postoperative (P<0.01).â‘¡To compare the blood vessel's number of preoperative and postoperative, there was obvious significant difference between preoperative and postoperative (P<0.01).â‘¢There was obvious significant difference between preoperative and postoperative in blood vessel typing's of medial femoral circumflex artery's upper trunk(P<0.05).â‘£There was obvious significant difference between preoperative and postoperative in femoral head's staining's extent(P<0.05).3.The function of hips (by Harris) after the operation, have been improved obviously.Conclusions:1.Interventional therapy can improve significantly the blood supply of fermoral head and promote the formation of bone, has significant short-term effection.According to the study about the changes of local blood vessel with Digital subtraction angiography (DSA), the changes of local blood vessel play an important part in Adult Avascular Necrosis of Fermoral Head.
