OBJECTIVES:1. To survey the clinical research and report about Henoch-Schonlein Purpura Nephritis (HSPN), master the situation of TCM syndromes study of HSPN to explore the problems.2. For the present situation and deficiencies, relevant studies carried out further explore the characters of TCM syndromes of HSPN, which lay the basis of pathogenesis and syndromes to guide clinical use of drugs and improving efficacy.METHODS:1. Index CNKI from 1979 to 2009 to obtain the necessary research literature. Analyze the situation of TCM syndromes study of HSPN and explore the problems by analysis on the basis of surveying the research literature on clinical about HSPN.2. For its problems, inspect hospital medical records of HSPN in children in Jiangsu Province hospital of TCM from 2002 to 2009, to collect related symptoms, signs and clinical examinations by retrospective clinical research methods. On the comprehensive analysis of indicators in the use of clustering analysis, principal components analysis, rank correlation analysis and other methods in SPSS 11.5, further explore the characters of TCM syndromes of HSPN, including the regulation of syndromes distribution, folders, the evolution and relationships with other relevant factors.RESULTS:1. At present the clinical physicians tend to type the HSPN into:syndrome of heat in blood, syndrome of qi deficiency, syndrome of yin deficiency, syndrome of dampness-heat blocking collaterals, syndrome of deficiency of both qi and yin, syndrome of wind-heat hurting collaterals and so on. However, there is not enough to grasp the dynamics of syndrome. Clinical physicians lack awareness of the relationship between the syndromes and the impact of some factors.2. The results of retrospective clinical study further indicate that TCM syndromes of HSPN of children mainly include syndrome of stirring blood due to intense heat, syndrome of hyperactivity of fire due to yin deficiency, syndrome of qi failing to control blood, syndrome of wind-heat hurting collaterals. Syndrome of stirring blood due to intense heat and syndrome of wind-heat hurting collaterals always combine with syndrome of blood stasis or dampness retention syndrome. Syndrome of qi failing to control blood and syndrome of hyperactivity of fire due to yin deficiency always combine with syndrome of blood stasis or syndrome of dampness-heat. Syndrome of blood stasis exists in every syndrome. Syndrome of wind-heat hurting collaterals turns into syndrome of stirring blood due to intense heat, syndrome of qi failing to control blood or syndrome of hyperactivity of fire due to yin deficiency. Syndrome of stirring blood due to intense heat turns into syndrome of qi failing to control blood, syndrome of hyperactivity of fire due to yin deficiency or syndrome of deficiency of both qi and yin. Syndrome of qi failing to control blood evolves into syndrome of hyperactivity of fire due to yin deficiency or yang deficiency syndrome. Syndrome of hyperactivity of fire due to yin deficiency evolves into syndrome of deficiency of both qi and yin. Syndrome of qi failing to control blood and course of HSPN, course of abnormal urine test, usage of Danshen injection are positive correlated. Syndrome of stirring blood due to intense heat and age, course of HSPN, course of abnormal urine test are negative correlation. The relation between the factors in this research and syndrome of wind-heat hurting collaterals, syndrome of hyperactivity of fire due to yin deficiency is not obvious.CONCLUSIONS:HSPN can be mainly typed into:syndrome of stirring blood due to intense heat, syndrome of hyperactivity of fire due to yin deficiency, syndrome of qi failing to control blood, syndrome of wind-heat hurting collaterals. Syndromes are combined and mutative in the treatment process. The related factors such as age, courses affect the formation of syndromes.
