BackgroundLumbar inter-vertebral disc protrusion, also called lumbar vertebral fibrosus ring crack or lumbar vertebral nucleus pulposus hernia symptom, refers to the lumbago due to the stimulation or press of the crack of the fibrosus ring or hernia of the nucleus pulposus to the nerve roots, vessels or the spinal cord, etc, after the occurrence of the regeneration of the lumbar inter-vertebral discs, accompanied with the radiating pain along the line of the sciatic nerves. In clinic, it is commonly seen among the middle-aged people, with the characteristics of high occurrence rate, long therapeutic session, prone to reoccurrence, etc.Fu's subcutaneous needling technique is a newly-developed therapy integrated the traditional acupuncture with the modern medicine, which has obvious therapeutic effects to multiple pain syndromes in clinic.ObjectiveTo observe the clinical therapeutic efficacy of Fu's subcutaneous needling therapy in the treatment of lumbar inter-vertebral disc protrusion symptom. By exploring the clinical value and the mechanism, it provides the theoretical basis for the treatment.Methods60 cases which were accordance with the inclusive criteria were enrolled and randomized into treatment group and control group, with 30 cases in each group. All the cases were applied with Fu's subcutaneous needling therapy in the treatment group and electro-acupuncture therapy in the control group. All the treatments were taken once every other day and 3 treatments make up a therapeutic session. According to the symptoms and signs before and after the treatments, with the JOA scale scoring table and VAS scoring was applied and analyzed, the clinical therapeutic effects between the two therapies in the treatment of lumbar inter-vertebral disc protrusion symptoms were compared.Results1. In the treatment group, among the 30 cases, after the treatments, no case was cured,8 were obviously improved,22 were improved and 0 was ineffective, with the total effective rate as 100%. In the control group, among the 30 cases, after the treatments, no case was cured,2 were obviously improved,25 were improved and 3 were ineffective, with the total effective rate as 90%. With the comparison of the therapeutic effects between the two groups, the rank test showed that Z=-2.535 and P=0.01≤0.01, with significant difference, indicating the therapeutic effects in the treatment group were better than that in the control group.2. With the comparison of the JOA scores before and after the treatments, the paired t-test showed significant difference (P<0.01). With the comparison of the JOA scores after the treatments between the two groups, the paired t-test showed significant difference between the two groups (t=3.470, p=0.002<0.01). With the comparison of the JOA scores difference before and after the treatments in two groups, the paired t-test showed significant difference (t=3.278, p=0.003<0.01). With the comparison of the JOA improvement rate after the treatments between the two groups, the paired t-test showed significant difference between the two groups (t=4.355,p=0.000<0.01).indicating that the improvement of the symptoms in the treatment group was superior to that in the control group.3. With the comparison of the VAS scores before and after the treatments in each group, the paired t-test showed significant difference (P<0.01) After the treatments, the paired t-test showed that the VAS scores in the treatment group were significantly different from that in the control group (t=-11.077, p=0.000<0.01). With the comparison of the VAS scores difference before and after the treatments in two groups, the paired t-test showed significant difference (t=7.059, p=0.000<0.01), indicating that there was remarkable improvement of pain symptoms in both two groups, with the treatment group better than the control group.ConclusionAfter the treatments of Fu's subcutaneous needling therapy, the symptom and sign improvement in the treatment group was better than that in the control group, with the remarkable therapeutic effects of checking pain, which was obviously superior to that in the control group.
