Clinical Study On The Treatment Of Knee Osteoarthritis In The Early And Middle Stage With Guan Jietong And Arthroscopic Debridement

Objective:Knee osteoarthritis(KOA) was a common, chronic, and progressive disease in the eldly. At Present, there was lack of cure method. Traditional Chinese Medicine thought that deficient of the liver and kidney, stagnation of qi and blood stasis, Wind-cold-dampness were the main pathogenesis. Guan Jietong can replenish the kidney and liver, nourish qi and promote blood circulation, remove wind and dampness. The features of Arthroscopic debridement were minimally invasive, quick recovery, simple, safe and effective.This study was to treat the disease in the early and middle stage with Guan Jietong and arthroscopic debridement, to observe the clinical outcome and concentration change of IL-6 and TNF-a in the serum. Meanwhile, the research analyzed the possible mechanism of treatment of knee OA with Guan Jietong and arthroscopic debridement. It would enrich and develop the theory of treatment of the knee OA with Traditional Chinese Medicine.Methods:60 eases were divided into two groups randomly:treatment group and control group. The treatment group was treated with Guan Jietong and arthroscopic debridement. The control group was treated with arthroscopic debridement only. Observe the concentration change of IL-6 and TNF-a in the serum before and two weeks after the treatment. Observe the Lysholm score before the treatment, two weeks after the treatment and three months after the treatment. All the data was processed by SPSS13.0 software package.Results: 1. In the two groups, the concentration of IL-6 and TNF-a in the serum was significantly lower after the treatments. The treatment group was lower than the control group with significant difference (P<0.01) two weeks after the treatment.2. the Lysholm score after two weeks treatment and three months treatment was notable higher than before in the two groups. No difference existed between the two groups after two weeks treatment. But after three months treatment, distinct differentiation appeared between the two groups with the the Lysholm score (P<0.01).Conclusion:The treatment with Guan Jietong and arthroscopic debridement worked effectively on the KOA patients, and reduced the concentration of IL-6 and TNF-a in the serum significantly. The treatment with arthroscopic debridement worked effectively too in short term but decreased the magic as time goes on. On the other hand, the treatment with Guan Jietong and arthroscopic debridement worked well in long term. It could effectively delay the time of artifical joint replacement, significantly alleviate the suffering of patients and economy burden, improve their quality of life.
