Objective: To investigate the different clinical efficacy between arthroscopic debridement and fibular osteotomy for the early or mid time medial compartment injure of knee osteoarthritis with a short term follow-up.Methods: Through the prospective randomized trial study that we will expect to collect 40 patients from January 2015 to December 2016 in our hospital with the diagnosis and operation treatment of knee osteoarthritis.All patients were randomly divided into two groups(Group A and Group B named).Each group contained 20 patients.Group A underwent the arthroscopic debridement therapy;Group B underwent the fibular osteotomy therapy.VAS,KSS and HSS score as well as complications rate,femur-tibia angle(FTA),The medial knee joint gap were measured separately when pre-operation and post-operation 1,3,6,12 months,all comparesions were done between two groups.Results: Totally,27 patients were included.Group A contains 12 patients,15knees;Group B contains 15 patients,18 knees.All patients acquired 12 to 20months’ follow-up visit.The mean follow-up in Group A and Group B were(15.91±2.61)months and(16.53±3.04)months.Two groups of patients with operation did not occur with post-operation complications such as the wound or joint infection,lower limb vein thrombosis,nerve injury.Compared to pre-operation,two groups of patients with higher postoperative HSS and KSS score,relatively lower preoperative VAS score,the difference statisticallysignificant(P < 0.05),Group A of postoperative FTA and medial joint gap compared with preoperative increase respectively(3.3±1.0)°,(1.53±0.50)mm while in Group B both changes had no statistical significance(P > 0.05).Comparison between groups: 1.VAS: at the 1th month follow-up,Group A had higher postoperative VAS score than Group B,at the 3th month they were approximative,while at the 6th and 12 th month Group A had lower postoperative VAS score and the difference statistically significant(P < 0.05).2.HSS:at the 1th month follow-up,the difference between had no statistical significance(P > 0.05)while at the 3th,6th and 12 th month postoperative HSS of A all higher than B,the difference statistically significant(P < 0.05).3.KSS:at the 1th and 3th month,the difference between two groups had no statistical significance(P > 0.05).At the 6th and 12 th month,Group A have higher score than B and the difference statistically significant(P < 0.05).Conclusion: After a short term follow-up,both fibular osteotomy and arthroscopic debridement had great early clinical efficacy for the early or mid time knee osteoarthritis,while the fibular osteotomy had been proved to significantly reduce the postoperative pain and facilitate the good recovery of joint function.Therefore,it is an effective method for the treatment of knee osteoarthritis and worth of promoting in clinical.
