Objective To investigate the chemotactic factors' changes and mechanisms in pantients with coronary heart disease (CHD). All cases were identified by Coronary angiograph measure,the MCP-1,RANTES and NFAT factors' level were measured by ELISA. Further, PCR-RFLP way was used to measure MCP-1-2518 G/A gene polymorphisms.Finally we analyzed the relation between the coronary arteriosclerosis changes and the chemotactic factors'concentration or MCP-1-2518 G/A gene polymorphisms in order to find new ways to predict the acute coronary syndrome (ACS).Method 360 cases were enrolled in our study, who underwent the Coronary Angiography (CAG), they were confirmed to have or not CHD. The cases were divided into coronary heart disease (CHD) group (n=300) and control group (n=60) according to the Coronary Angiography (CAG).CHD group was further divided into stable angina pectoris (SAP) group (n=120) and acute coronary syndrome (ACS) group (n=180) according to the clinical property include unstable angina pectoris(UAP,n=60),acute myocardial infarcation (AMI,n=120). The severity and extent of coronary lesions was analyzed by CAG and typified by means of Gensini coronary score system. Linked immunosorent assay was used to measure the MCP-1, RANTES and NFAT factors' concentration.PCR-RFLP way was used to measure MCP-1-2518 G/A gene polymorphisms.Results1 Significantly increasing of MCP-1,RANTES,NFAT,glycemia ,LDL-C, hs-CRP concentration was observed in ACS group comparde to the SAP group and the control group,while the HDL-C concentration decreased(P<0.05).2 Positive correlation were seen between the Gensini score of coronary artery lesions and the factors of MCP-1,RANTES,NFAT, glycemia, LDL-C, hs-CRP by Linear Regression analysis,while the HDL-C factor was seen negative correlation(P<0.05).3 The effect of MCP-1,RANTES,NFAT binary logistic way to predict ACS is equvialent to the general hs- CRP binary logistic way.But the effect of comibantion detection MCP-1 and RANTES binary logistic way to predict ACS is much better than the general hs- CRP binary logistic way.4.70.7 percent coronary heart disease patients were identified diabetes mellitus or pre-diabetes.Significantly increasing of RANTES and MCP-1 concentration was observed in diabetes mellitus group comparde to the pre-diabetes group and the normal group (P<0.05).Hyperglycemia was positively correlated with MCP-1 concentration by liner regression analysis(Spearman's Correlation, r=0.69,P<0.05). Hyperglycemia was positively correlated with RANTES concentration by liner regression analysis(Spearman's Correlation,r=0.61,P<0.05).5 Significantly increasing of MCP-1 gene polymorphisms GG gene and G allelic gene was observed in ACS group compared to the SAP group and the control group (P <0.05).The MCP-1 concentration was observed significantly increasing from AA gene group.AG gene group to GG gene group (P<0.05).Conclusions1 The effect of comibination detection chemotactic factors to predict ACS is better compared to general hs-CRP measurement, multi-chemotactic factors' combination dection maybe come to markers of predicting the unstability of plaques. 2 Hyperglycemia was positively correlated with coronary heart disease,MCP1 or RANTES concentration was positively correlated with hyperglycemia, positive correlation were seen between theglycemia, RANTES or MCP-1 factor and coronary heart disease, MCP1 and RANTES factors plays important roles between diabetes and coronary heart disease.3 NFAT was positively correlated with coronary heart disease,NFAT factor play some roles in CHD and ACS, yet the effect of NFAT factor to predict ACS is lower compare to general hs-CRP measurement, NFAT factor may not be an ideal marker predicting the ACS.4 MCP-1 gene ploymorphisms is positive regulators of coronary artery lesions.The GG gene and G allelic of MCP-1-2518 G/A polymorphism can increase the amount of MCP-1 than its' AA gene and A allelic in cononary heant disease.
