Study Of Professor Yang Minghui's Clinical Experience On Recurrent Aphthous Ulcer Treatment

Objective:This study integrated professor Yang's TCM clinical experience, exspecially on RAU and correlative literatures study of RAU. We observe and evaluate the effect of use of Jungan collutory to treat RAU based on multi centers and randomized controlled trial (RCT) for providing evidence to popularize and research this recipe of professor Yang's advantageously.Methods:To interpretate professor Yang's TCM clinical experience, exspecially on RAU. There were 200 RAU cases collected from six clinical centers. The cases were randomized into treatment or control group through method of random digits table. The treatment group treated in Jungan collutory, as compound chlorhexidine collutory in the control group. The use method of the two drugs were consistent: 20ml per use, mouthwash it after water gargle, mouthwash 2 to 5 minutes, then, keeping from eating and drinking for 30 minutes, three times daily,5 days were total observational course. Observational indexes contained ulcer diameter, pain index, total ulcer number, average duration, and total effect, et al. SPSS 13.0 software was used to analyze the data. Results:After treatment, the total effect of treatment group (94.0%) was superior than control group (79.0%), the difference was statistical significance (P<0.05), which indicated the effect of Jungan collutory on RAU treatment was better than compound chlorhexidine collutory. All of the ulcer diameter, pain index, total ulcer number and average duration of treatment group was superior than control group, all the difference were statistical significance (P<0.05), which indicated Jungan collutory could improve ulcer diameter, pain index, total ulcer number and average duration more effective than control group. All of the ulcer diameter, pain index, total ulcer number and average duration of the both groups had been ameliorated, all the difference were statistical significance (P<0.05), which indicated both the drugs were effective on improving RUA.The total effect of syndrome of accumulation of heart and spleen of treatment group was superior than control group, the total effect of syndrome of fluid retention due to spleen deficiency of treatment group was superior than control group, both the total effect of the syndromes of flaring up of stomach fire and syndrome of yang deficiency of spleen and kidneys of the two groups were similar, which indicated Jungan collutory obtained excellent effect based on correspondence of recipe and syndrome, and the total effect was superior than not correspondence of recipe and syndrome and control group. The total effect of syndrome of yin deficiency of liver and kidneys of treatment group was superior than control group, the effect of treatment group was superior. A few adverse effects occurred in the both groups, although, which were managed effectively. Conclusions:Professor Yang Minghui's clinical experience in traditional Chinese medicine is nourish. He enhances to integrate both differentiation of diseases and syndromes for RAU treatment, as well as strenthens to base on fire and external therapy method of traditional Chinese medicine for RAU treatment. This clinical study demonstrates the use of Jungan collutory to treat RAU was better than compound chlorhexidine collutory, including in amelioration of ulcer diameter, pain index, total ulcer number, average duration, and total effect. Especially, Jungan collutory obtained excellent effect based on correspondence of recipe and syndrome, the total effect of which was superior than not correspondence of recipe and syndrome and control group. The evidence of this RCT supports and suggests that the use of Jungan collutory for RAU treatment could deserve to popularize and research advantageously in clinical practice.
