A recent study found a novel type of CD4~+ effector T cell, different from Th1 and Th2 CD4~+ T effector cells, specificly producted the interleukin 17, was known as Th17 cells [1]. It played an important regulatory role in many autoimmune diseases and inflammatory diseases. This study were designed to suggest the changes and relevance of IL-17, hs-CRP, WBC count and cTNI by detecting them in patients with ACS, and to discuss the significance of the pathogenesis in patients with ACS and the instability of atherosclerotic plaque. Objective To observe the changes of the levels of IL-17, hs-CRP, WBC count and cTNI in patients with ACS, and whether there was the correlation or not among them, and to investigate the relationship between them and the instability of plaque as well as the pathogenesis of ACS.Methods 113 patients were divided into 4 groups: 40 cases attacked in 48 hours with acute myocardial infarction (AMI), 31 cases with unstable angina (UA), 18 patients with stable angina pectoris (SAP), and 24 patients with chest pain syndrome (CPS). All were took blood immediately after admission from venous, and centrifugal samples stored in -80℃refrigerator to detect the levels of IL-17 in serum by ELISA, meanwhile to detect the levels of WBC count, hs-CRP and cTNI in peripheral blood. Results In peripheral blood the levels of IL-17 in AMI (37.04±9.92) ng L-1, and UA (30.11±5.67) were significantly higher than the SAP (20.95±5.08) ng L-1 and the CPS [( 15.81±7.65) ng L-1, (P <0.01)]. Levels of hs-CRP in AMI and UA [ (39.21±14.42) mg L-1, (11.57±3.71) mg L-1] were significantly higher than the SAP (3.53±4.45) mg L-1 and the CPS groups(2.94±3.97)mg L-1 (P<0.05). WBC count [(9.52±3.85) * 10~9·L-1] in patients with AMI were significantly higher than the UA group and the CPS group which were (6.43±1.64) * 10~9·L-1 and (6.29±2.04) * 10~9·L-1 (P <0.001 ). IL-17 and hs-CRP were significanrtly correlated (r=0.364, P <0.001), WBC count and hs-CRP were significanrtly correlated (r=0.578, P <0.001), WBCcount and IL-17 were significanrtly correlated (r=0.302, P = 0.004). IL-17 and cTNI were also significanrtly correlated (r=0.539, P <0.001).Conclusions The levels of IL-17 and hs-CRP were significantly elevated and were positively correlated with the level of cTNI; WBC count in AMI group was obviously higher than others, and was positively correlated with the levels of IL-17 and hs-CRP We suggested IL-17, WBC count and hs-CRP were associated with the instability of atherosclerotic plaque, and also involved in the pathogenesis of ACS; IL-17 was a proinflammatory an it involved in the inflammation of AS by the IL-17-CRP way to induce and stimulate the secretion of the inflammatory mediators; The singal system of IL-17-CRP might be the important components of inflammatory in the mechanism of plaque formation and cardiovascular diseases.
