Objective: To investigate effect of different programs of preconditioning with hyperbaric oxygen(HBO) on renal ischemia-reperfusion injury in rats.Methods:Fifty SD rats buying from Anhui medical university were randomly divided into five groups: group S, sham operation (n=10);group IR: rats I/R injury(n=10);group H1-3.The animals were pretreated with 100% O2 at 2.5ATA 1h/d for 4(group H1),8(group H2),or 16(group H3) consecutive days respectively 24h before renal I/R. In surgery group,ischemic acute renal failure model was established by bilateral clipping of renal arteries for 45min and reperfusion for 24 hours.Observe the color of renal when it from dark red into bright red that means reperfusion success, keeping after close abdominal cavity and injecting the rat with the same size saline through intraperitoneal 24 hours before the model was established.In group sham,find renal arteries but not clip. HBO+I/R groups were same as group model.The animals were then killed and serum content of maleic dialdehyde(MDA),blood urea nitrogen(BUN),serum creatinine(Scr) were examined.Structure of the kidney was observed under optical microscope.The expression of TNF-αand IL-6 Was detected by ELISA.Results:The levels of IR group BUN, Scr, MDA, TNF-α, IL- 6 were more than S group's (P < 0.01)and in IR group's epithelial cells from the tube were varied by extention of necrosis, edematous and inflammatory cells infiltration , vessels penetrations for shielded enclosures, glomerular atrophy;In H1, H2, H3 groups, the levels of BUN, Scr, MDA, TNF-α, IL- 6 were lower than IR group(P<0,05);In H1,H2,H3 groups the epithelial cells were only swelling, a small number of inflammatory cells were infiltration ;The expression levels of TNF-α, IL- 6, MDA of H3 group were lower than H2 group (P< 0.05)Conclusions: Preconditioning with 4,8 or 16 d of HBO reduced the injury of renal function and the pathological extent of renal tubular injury in rats with ARF and reduced the injury of TNF-αand IL-6.But preconditioning with 16 and 8 days of HBO was different. In morphological changes of renal tissue, preconditioning of HBO reduced acinar necrosis,edema, leukocyte infiltration. Red blood cells in glomerulus were seen in IR group.
