【Objective】1.To investigate the features of the disturbance of the cognitive function in the patients with complex partial seizures .2. To investigate the correlation between the cognitive function and proton magnetic resonance spectroscopy (1H-MRS).【Methods】45 patients with complex partial seizures ,40 patients with generalized tonic-clonic seizures and 20 healthy controls were examined by the following means: memory function examination by the clinical memory scale, intelligence function examination by Ravin's standard progressive Matrices (R'SPM) and 1H-MRS scan in the bilateral hippocampal. Memory quotient (MQ), Intelligence quotient (IQ), the levels of NAA, Cho, Cr and the ratios of NAA/Cr+Cho were compared between these groups. Meanwhile, the correlation coefficients were calculated in patients with complex partial seizures.【Results】1.The scores of pointing memory, associational learning, image free memory, association memory of portrait and MQ excepts insignificant figure recognition were significantly lower in patients with complex partial seizures than those in the control group (P<0.05); the scores of Ravin's standard progressive Matrices and IQ were noticeably lower in patients with complex partial seizures (P<0.05) than those in the controls. 2. In patients with complex partial seizures ,the MQ and IQ of the group of patients who took antiepileptic drugs (AEDs) were significantly lower than the group who didn't take AEDs , the MQ and IQ of the group of patients whose frequency of seizure of the disease exceeded 4 per month were significantly lower than the group whose frequency was lower than 4 per month (P<0.05).3.No significant difference of IQ and MQ was showed in the two groups of patients with complex partial seizures and patients with generalized tonic-clonic seizures(P>0.05).4.The levels of NAA, Cho, Cr and the ratios of NAA/Cr+Cho in the group of patients with complex partial seizures were significantly lower than those in the controls (P<0.05). 5.Patients with complex partial seizures showed remarkable correlation between MQ and the lever of NAA, MQ and the ratio of NAA/Cr+Cho (P<0.05), IQ and the lever of NAA, including IQ and the ratio of NAA/Cr+Cho (P<0.05), while MQ and the lever of Cho, including IQ and the lever of Cho (P<0.05), is in inverse proportion. However, there was no correlation between MQ and the lever of Cr, including IQ and the lever of Cr (P>0.05).【Conclusion】1.The patients with the complex partial seizures do have the disturbance of short-term memory and the decreasing ability of abstraction, maturity judgment and reasoning. Both the high frequency of seizure and AEDs could affect the cognitive function. 2.The correlation between cognitive function and the levels of NAA, Cho, Cr ,the ratios of NAA/Cr+Cho indicated that 1H-MRS can be used as an important objective examination to check the cognitive function of the patients with complex partial seizures in early stage. Working with Neuropsychology examination, it can detect the disturbance of the cognitive function in the patients with complex partial seizures precisely in early stage.
