| ObjectivesXerosomia is a serious and ubiquitous sequelae in the patients with NPC after conventional radiation therapy. It has a significant impact on the patient,s quality of life of life This study was designed to investigate the severity of xerostomia and its impact on the quality in the patients with NPC after conventional radiation therapy;And to observe the effect of Chinese herbal medine- Zengye fluid on the injured saliva gland on the patients with NPC after radiation therapy.Methods60 cases with NPC receiving intial treatment with radiotherapy were randomly assigned into two groups:treatment group (n=30) and control group (n=27). The treatment group was treated with the method of Zengye fluid combined with conventional radiation therapy, and the control group was treated with radiation therapy alone. The injuried oral mucous of patients at DT 0,10, 30,40,50,60Gy,70Gy as well as the pH in the mouth before and afrer radiation therapy was observed. Moreover, apply the R.O.I sequence software to quantititive evaluation of salivary gland funtion before and afrer radiation therapy by SPECT radioactive dynamic imaging; meanwhile all the patients were surveyed by interview.A questionnaire and a visual analog scale(VAS) were used in analysis of xesromia and xerostomia-ralated problems.ResultsIn the process of radiotherapy, all cases occured the acute radiation oral mucositis, related with the radiation therapy dose. The condition of injuried oral mucous of treatment group was better than that of control group (P<0.01). When DT≤30Gy, both I and II degree of injuried oral mucous between two groups was statistically significant (P<0.01). And bothⅡandⅣdegree of injuried oral mucous was statistically significant (P<0.01) at DT 70Gy, what was not of III degree when DT40Gy. With the dose increasing, the UI and EI of saliva gland in both of the two groud decreased, while El decresed worse than UI. In the treatment groud, the decrese ofUI,EI was 29.1%,44.3%after radiation therapy; sepectively what in the control groud was 44.3%%,74.1%. The treatment groud is surpior to the control one. While about the pH in the mouth, either groud was not lower than normal level (6.5~7) and it was related to the occurrence of the acute ridia active.The incidence of xesromia was found in all 60 patients. Those from moderate to high degree in two grouds comes to54.6% and 77.9%. It revealed that the QOL of treatment group was improved and was superior to control group. The VAS about xesromia indicats statistical difference between two groups (P<0.01).ConclusionsAll the 60 patients with NPC in study experienced different degrees of xesrostomia.Xerostomia has a sinificant impact on the patient, s speech, deglutition, sleep, and increase the morbidity rate of the mouth. SPECT radioactive dynamic imaging is a sensitive and useful method in determining the salivary gland function of NPC patients who were treated by radiotherapy. As to salivary gland function damage of NPC patients who were treated by radiotherapy, the parotid gland function damage is primary. And the excretion function damage is more serious than the ingestion function damage of parotid gland. The Chinese herbal medine-Zengye fluid Can postponed the acute radiation oral mucositis, released the degree of dry mouth and improve quality of life in patients with NPC of ter conventional radiation therapy, Furthermore, this therapy has such practical potentiality to promote. |
