Background and Objective:The incidence of gastric cancer in China is higher than that of any other country over the world..Surgical treatment is the most effective and the only way to cure the early gastric carcinoma. But many of these patients were diagnosed at advanced or late stage.So the treatment outcome is not satisfactory.To achieve the purpose of effective treatment, comprehensive treatment must be made. Because the target of cancer chemotherapy is also universally exist in normal tissue cells, Therefore, Chemotherapy has a serious side effects to the body. So find a new non-surgical intervention for the treatment of advanced gastric cancer and to prevent the gastric carcinoma's recurrence and metastasis have a very important clinical significance. Molecular targeted therapy is the focused area in the field of cancer treatment currently.Overexpression of epidermal growth factor receptor (EGFR) has been correlated with tumorgenesis.The critical role the EGFR plays in cancer has led to an extensive search for molecular targeted therapy of EGFR .EGFRvⅢ(epidermal growth factor receptor variantⅢ) is the most common mutant form of EGFR and hasn't yet been detected in normal tissues. So it is regarded as a tumor-specific gene and offers a new way for targeted therapy of cancer. In order to provide strong theoretical basis for patients of gastric cancer in tailored comprehensive treatment plan and elucidating the prognosis and molecular targeted therapy, the experiment to explore its relation with the genesis and development of gastric carcinoma and its correlation with clinicopathological parameters.Materials and methods:1.Materials:58 cases of gastric adenocarcinoma tissues were obtained from patients who exa- mined by gastroscope at Endoscopic Center of Henan Provincial Hospital from March 2010 to June 2010. Excluded the cases of esophagogastric junction carcinoma.which were confirmed by Pathology .The patients included 36 men and 22 women. The mean age was 64 years,range from 37 to 83 year. 40 casee received radical resection and 18 cases gave palliative treatment.All of the cases hadn't been offered chemotherapy or radiotherapy before surgery.Well or moderately,poorly differentiated samples were 24 and 34.24 cases had lymph node metastasis.And 34 cases had no lymph node metastasis.According to the standard of TNM staging, the number of patients with stage 0,I,II,111,IV were 8, l9,10,10,11,respectively.As for infiltrative depth,18 patients had invad- ed through the serosa.20 cases of normal gastric mucosa were obtained from healthy volunteers who received gastroscopy biopsy over the same period(All confirmed by pathology).2.methods:immunohistochemistry (MaxvisionTM2) was used to detect the expression of EGFR and EGFRvⅢprotein in 58 gastric carcinoma tissues as well as 20 normal gastric tissues specimens from non-cancer patients was performed by immunohis- tochemical staining.3.SPSS l7.0 statistical software was used to analyze the data.andχ2 test was used to assess the association between the irnn1unohistochernical status and the clinicopathologica characteristics.Results:Positive expression rate of EGFR and EGFRvⅢin gastric carcinoma tissues were higher than normal gastric tissues (P<0.01), their positive expression rate were 65.5%,58.6% and 30%,0%. Their expression were irrelated with gender, age, size,and site of gastric carcinoma (P>0.05),but were closely related with the degree of differentiation,depth of invasion node metastasis and clinical stages and distant metastasis of gastria cancer patients (P<0.05).The radical resection rate in gastric cancers with both positive EGFR and EGFRvⅢwas 37.9%.significantly lower than that with both negative EGFR and EGFRvⅢ(P=0.000).Conclusion:EGFR and EGFRvⅢhave been found in many gastric carcinoma tissues.Over- expressed EGFR and EGFRvⅢmay be really related to initiation and progression of gastric carci- noma.These results were indicated that,whereas,EGFR and EGFRvⅢcould be regarded as the marker for predicting the initiation and prognosis of human gastric carcinoma.And the specificity expression of EGFRvⅢcan make it become the ideal target treatment of gastric carcinoma.
