| Objective:To explore the reasons and treatments of nonunion of clavicular fracture, analysis the efficacy of large iliac bone grafting and anatomical plate fixation treatment of clavicle fracture nonunion.Methods:28 cases of clavicular fracture nonunion treated by large iliac bone grafting and anatomical plate fixation treatment in Shandong University of Traditional Chinese Medicine Affiliated Hospital over the past 3 years (2006.10-2009.10),20 males,8 females, aged from 11-60,36.50±14.44 in average. Outreach position fixed for 4-6 weeks postoperation, X-ray re-inspected regularly. Guide the patients to do exercises systematically, comprehensively and correctly. Remove the plate when the X-ray showed the fracture healing. Association of the United States, shoulder and elbow shoulder score (ASES score) as evaluation criteria.Results:28 cases were all followed up for 9-19 months,14.57±2.53 months in average. All fracture healing, the time was 8-18 weeks,12.85±2.23 in average. No infection, screws broken, plate fracture or other complications. Preoperative score was 39.21±15.59,0 excellent, 1good,5 medium,22 poor. The score was 87.32±8.60 in average after fracture healing,13 excellent,13 good,2 memiun,0 poor.Conclusion:The treatment of open reduction surgery, anatomical plate, iliac bone grafting of clavicular nonunion, has benefits fixed and reliable stability, small local blood supply destruction, good collar bone shape maintainance, fracture healing conducive and early shoulder joint movement, is one of the effective treatments of clavicle fracture nonunion.
