Objective To evaluate the value of gastroesophageal reflux disease questionnaire (GerdQ) in the diagnosis of gastroesophageal reflux disease(GERD), meanwhile,studied the clinical characteristics of patients with GERD,explored a simple, easy method for the diagnosis of GERD groups.Methods1.objects and groups Patients with heartburn or reflux symptoms was studied in Department of Gastroenterology the First Affiliated Hospital, Shanxi Medical University, using strict inclusion and exclusion criteria, a total of 350 cases patients were selected, with gastroscopy and PPI test as a diagnostic criteria of GERD,Gastroscopy diagnosis of reflux esophagitis(RE) patients or PPI test was positive for the GERD group, a total of 221 cases;Gastroscopy is not diagnostic of reflux esophagitis and the PPI test was negative for the control group, a total of 129 cases.2.GerdQ survey All selected patients completed uniform questionnaire-GerdQ,recorded the score of frequency of each symptoms within one week, including A1(heartburn),A2 (reflux), B1 (upper abdominal pain), B2 (nausea), C1 (sleep disorders), C2 (OTC medication), the symptom score of A1, A2, C1, C2 increased with the increasing frequency, the symptom score of B1, B2 decreased with increasing frequency, the sum of each symptom score is the score of patients with GERD. Analyzed the distribution of GerdQ score and understood the symptoms characteristics of patients with GERD.3.statistical analysis Analysed the gender difference between the GERD group and the control group with a X2 test, analysed the correlation between reflux esophagitis LA grade and GerdQ score by Spearman test, treatmented age and the score distribution with non-parametric Mann-Whitney test, P < 0.05difference was of statistically significance. Calculated diagnostic threshold for GERD through the ROC curve.Results 1.General Information 350 selected patients, male is 177 cases (177/350, 50.6%), female was 177 cases (173/350, 49.4%),male to female ratio was 1.02:1, the average age was(46.37±11.27) years old, female was 113 cases in GERD group(113/221, 51.1%), female was 60 cases in control group, 60 (60/129, 46.5%), the gender had not significant difference (P>0.05) between the GERD group and the control group with a X2 test.testing between GERD and control groups no significant gender differences (P> 0.05); the average age of GERD group was (47.59±10.60) years old, the mean age of control group was (44.27±12.10) years old,age difference was of statistically significant between the two groups(*P<0.05),with non-parametric Mann-Whitney test, the average age of GERD group was older than the average age of the control group.2.GerdQ score Taking 10 as GerdQ threshold score for GERD, we got the maximal Youden index of 0.368, with sensitivity of 78.7% and specificity of 58.1%, the area under the receiver operating characteristic curve (ROC) was 0.733, the positive predictive value was 67.2%, and the negative predictive value was 45.2%. And in GerdQ scores, GERD group Total and the A1 (heartburn), B2 (nausea), C2 (OTC drugs) score compared with the control group, there were statistically significant differences (* P <0.05, ** P <0.01 ),while the A2 (reflux), B1 (upper abdominal pain) and C1 (sleep disturbance) score showed that there existed no statistical significanc (P> 0.05) between the GERD group and the control group.3.The relationship between RE and GerdQ reflux esophagitis LA grade had not correlation with the GerdQ Scores.Conclusions1.GerdQ can diagnose GERD by the symptoms of GerdQ, when GerdQ threshold score is 10 , GerdQ is of the greatest diagnostic value for GERD.2.GerdQ can assess the quality of life of patients by the symptoms of GerdQ.
