| Objective:Carbetocin is a new long-acting oxytocin, can cause rhythmic contractions of the uterus, the contraction in the original basis, increasing the frequency and increased uterine tension. In recent years, we can see the report about the application of Carbetocin during cesarean section to prevent postpartum hemorrhage, but Carbetocin during vaginal delivery in prevention of postpartum hemorrhage was little studied. The main purpose of this study is that observe the clinical efficacy on preventing postpartum hemorrhage caused by uterine inertia by using carbetocin during cesarean section and vaginal delivery.1. The amount of bleeding intrapartum (during surgery), within 2 hours and 24 hours after the delivery(after surgery) for carbetocin group and oxytocin group was compared.2. The hemoglobin change of carbetocin group and the oxytocin group of pregnant women before the delivery and 24 hours after the delivery was compared.Methods:400 cases of normal term delivery in pregnant women were divided into two groups,200 cases of vaginal delivery group,200 cases of cesarean section group, each group was divided into carbetocin group and oxytocin group. The carbetocin group was given carbetocin 100μg into the pot after the delivery of the fetus, while for the oxytocin group, oxytocin lOu was given into the pot or injected in the uterus immediately after the delivery of the fetus, 10u into intravenous infusion last to 2 hours after the delivery.1. Collecting the bleeding intrapartum (during surgery), within 2 hours and 24 hours after the delivery(after surgery) for carbetocin group and oxytocin group.2. Testing hemoglobin of the pregnant women before the delivery and 24 hours after the delivery.3. All the datum were analyzed by statistics software SPSS 11.5. Group-samples T test was used to compare between two groups.Results:1. Comparison of the general state:There were no significantly differences in pregnant women'age, weight, gestational age and neonatal weight between the carbetocin group and the oxytocin group in the vaginal delivery group and the cesarean section group.2. Comparison of the amount of bleeding of uterus:In the vaginal delivery group and the cesarean section group, the amount of bleeding intrapartum (during surgery), within 2 hours and 24 hours after the delivery(after surgery) for carbetocin group were significantly less than the oxytocin group. There were significantly differences between the carbetocin group and the oxytocin group(P<0.05). 3. Comparison of the decreased hemoglobin:In the vaginal delivery group and the cesarean section group, the decreased hemoglobin of 24 hours after the delivery for carbetocin group were significantly less than the oxytocin group. There were significantly differences between the carbetocin group and the oxytocin group(P<0.05).Conclusion:1. Carbetocin for the prevention and treatment of postpartum hemorrhage are better than the role of oxytocin.2. Carbetocin is better for the prevention of postpartum hemorrhage than the treatment.3. Carbetocin is safe either cesarean section or vaginal delivery for the prevention of postpartum hemorrhage. |
