Objective:To observe the surgical results of ossification of the posterior longitudinal ligament (OPLL) treated by anterior vertebral subtotal vertebrectomy,bone graft fusion and internal fixation,and analyse the correlative affecting factors.Method:40 cases of OPLL treated by the anterior cervical approach in recent 6 years were collected in this study.According to JOA 17 score method,score of preoperation,one-week postoperation and the end of follow-up and the increase of intervertebral height were written down to analyse the surgical results.Cases were divided into two groups(excellent group and good group)which were according to the rate of melioration to analyse the factors that affected the surgical results.Result:JOA scores of preoperation,postoperation and the end of follow-up were increased obviously.lt showed that the surgical results of anterior cervical decompression,bone grafting fusion and internal fixation treating cervical spondylotic myelpathy were opositive.patients in the eximious group showed yonger ages,greater preloprative JOA scores and shorter duration symptoms, The postoperative improvement rate of ISI (Increased signal intensity) group was superior to no ISI group.Conclusion:The postoperation and long-term results of surgical treatment on OPLL by anterior vertebral subtotal vertebrectomy, bone graft fusion and internal fixation are definite.The patient ages,duration of symptoms and preoperativre JOA score can influence the treatment results.The postoperative improvement rate of ISI (Increased signal intensity) group was superior to no ISI group.The spinal cord signals change has certain ualue to judge the prognosis sufficient preoperative preparation.
