Objective:This paper analyzes the unstable scapular fracture clinical effect of surgical treatment, to investigate the early diagnosis, the classification, adaptation levy of surgical treatment, surgical approach, safety and the precise nature of long-term efficacy of the unstable scapular fracture, and to provide the proofs to guide the diagnosis and treatment in clinic practice. Methods:The study group includes twenty-five cases for scapular fracture. Views of X-ray and three—dimensional CT image reconstruction were used to conform their injures before surgery. According to the Hardegger's classification for scapular fracture, all of them were classified, and were conform to adaptation levy of surgical treatment. Depending on the fracture site, the patient were used the appropriate surgical approach. Operative treatment included reconstruction plate or calcaneal plate with K—wire and wire fixation, etc for fixation of scapular fracture. Results:The twenty-five cases for scapular fracture are to achieve anatomic reduction, fixed satisfied, no postoperative wound infections after the operation. A11 cases were followed up. A11 got bony healing and a good functional activity of limb activity. According to the Hardegger evaluation, all cases were divided into 4 groups, excellent cases, good cases, fair cases and poor cases, there were 16 excel lent cases,5 good cases,3 fair cases and 1 poor cases. The excellent and good rate is 84.0%. Conclusions:Three—dimensional CT reconstruction may help use to make the diagnosis, to understand the fracture patterns and the degree of displacement, and choose the fitting treatment. Most cases of scapular fracture can be healed by conservative treatment, but there are more complications than some by operative treatment for the unstable scapular fracture. Adolescent are the majority of all cases, the criterion of operative treatment need to be broadened for good therapeutic effects. Some cases especially the nonsurgical group can't take exercise in time, which seriously affect the treatment.
