| Object This study aimed to analyze the expression of tumor necrosis factor-alpha,matrix metalloproteinase-2 and p16 in human middle ear cholesteatoma and to investigate the correlation between its expression and the ability of erosion of cholesteatoma.Methods Specimen from Shanxi Medical University first clinical hospital otolaryngology, in collection cholesteatoma tympanitis patient surgery fresh specimen, quickly fixed, made of paraffin, while observing the patients' bone damage. The immunohistochemical technology and the computer image analytic methods were used to detect the expression of tumor necrosis factor-alpha,matrix metal proteinase-2 and p16 in 30 examples of cholesteatoma. Meanwhile, to observe the expression of the three and correlation of the degree of bone destruction, and take 10 cases of ear canal skin as a control to explore the possibility of cholesteatoma mechanism of bone destruction.Result1.The percent of positive expression of TNF-α, the MMP-2 and p16 in middle ear cholesteatoma were 70.0%,60.0%,63%, as compared with the normal ear canal skin, there were significant differences in expression (P<0.05.). TNF-a, MMP-2 expression of positive results for the brown granules, mainly located in the cytoplasm, TNF-a expression in cholesteatoma epithelium and subepithelial connective layer of stromal cells, MMP-2 layers of cells in cholesteatoma epithelium expressed in both basal expression the strongest level of the spine, granular layer and stratum corneum cells is weak.The staining of P16 were located in the nuclei and cytopiasm of cells,mainly in the nuclei, expressed in mainly in the prickle and granular layers of cholesteatoma epithelium layers of cholesteatoma epithelium.)2.Cholesteatoma of TNF-a and MMP-2 expression were positively correlated (r=0.694, P <0.01); and TNF-α, the MMP-2 and p16 expression were no significant correlateion (all P>0.05).3.It showed statistically significant correlation between expression of TNF-a and the ability of erosion of middle ear cholesteatoma (P<0.01).There was a correlation between the expression of MMP-2 and the ability of erosion of middle ear cholesteatoma too(P<0.01). But there was no correlation between the expression of p16 and the ability of erosion of middle ear cholesteatoma (P>0.05)Conclusion1.The TNF-α, MMP-2, and p16 in the adult middle ear was highly expressed, compared with the skin of external auditory of healthy people. TNF-α,MMP-2 expression is correlated, suggesting that the interation between the two, the latter may be induced by the former factors.2.There was correlation between the expression of TNF-α,MMP-2 and the ability of erosion of middle ear cholesteatoma. It means that TNF-a,MMP-2 play a key role in the hyperproliferation middle ear cholesteatoma. But there was no correlation between the expression of p16 and the ability of erosion of middle ear cholesteatoma (P>0.05).3.Cholesteatoma TNF-α, the MMP-2 expression and high degree of bone destruction index for clinical detection of conservative treatment, prognosis for reference.)4.The high expression P16 in middle ear cholesteatoma may be a cause of the inhibiting proliferation epithelium hyperplasia. |
