| Objective:To study the diagnosis to the high risk group of the Cardiogenic brain embolism (CBE) and the Antithrombotic treatment strategies.Method:To analysis the treatment process of a patient occurring CBE with RHD and concomitant atrial flutter, and to carry out related literature review.Results:The patient was hospitalized with diagnostic rheumatic heart disease, chronic heart failureâ…¢degree (full heart failure), arrhythmia (atrial flutter) hypertension, diabetes. During hospitalization, he was given cardiac, diuretic, lower blood pressure, lower glucose and symptomatic treatment. The results was the RHD occurred to the patient after one week. And thought Echocardiogram confirmed the presence of cardiac emboli, so consider it is a CBE case. The patient in this CBE case was cured with anticoagulant therapy after absolute risk assessment and bleeding risk assessment, his condition had markedly improved, no bleeding complications and short-term recurrence were followed for six months Follow-up visit.Conclusion:1.The patient who with the risk of CBE should have CBE risk assessment.2.To the patient with high risk elements, the effect to carry anticoagulant therapy is better than antiplatelet therapy.3.For anticoagulation, it should choose low intensity anticoagulation, INR:(1.6-2.5),the consequence is safe and effective. |
