| Objective:To retrospectively analyze the application of anti coagulation therapy in patients with cardiac cerebral embolism in our hospital,and to study the effect of low molecular weight heparin on the prognosis of atrial fibrillation combined with stroke in the acute phase.Understand the regulatory rate of antithrombotic therapy in patients with cardiac cerebral embolism.Methods:250 cases of atrial fibrillation associated with stroke patients were selected.Cases were divided into aspirin group(A group,71 cases)and low molecular weight heparin group(B group,179 cases)according to the antithrombotic treatment.Two groups of other treatment drugs were given according to the guidelines.At the time of admission and discharge,patients underwent NIHSS score and cranial CT or SWI examination to determine the condition,record the treatment of craniocerebral hemorrhage after treatment and determine the clinical effect of the two groups.According to the antithrombotic treatment plan,it was assessed whether the guidelines were met.Results:The regulatory rate of Group A and Group B was 22 cases,31.0%,76 cases,42.5%,and there was no significant difference in two groups(P=0.094).The NIHSS scores at the time of discharge from two groups were lower than those at admission,P was 0.002 and 0.000,and the difference was statistically significant.There was no significant difference in the NIHSS scores and efficacy between the two groups at admission and discharge time(P was 0.181,0.558 and 0.089).After treatment,the rate of cerebral hemorrhage in the Group A was 23.8% lower than that of the Group B(50.7%),and the difference was statistically significant(P=0.029).The staxis rate in group A was 80.0%,and the staxis rate in group B was 36.1%.There was no significant difference in the degree of intracranial hemorrhage(P=0.167).Conclusion:Low molecular heparin is effective in treating acute atrial fibrillation and stroke,but the rate of intracranial hemorrhage is relatively high.And there is no difference in the degree of intracranial hemorrhage.It is not recommended to use heparin for anticoagulant therapy in acute phase of atrial fibrillation combined with stroke.If antithrombotic therapy is applied,antithrombotic therapy according to the guidelines is required. |
