Objective:To compare the clinical effectivity of Holmium laser resction of bladder tumors(HOLBT) with transurethral plasmakinetic resection of bladder tumors(PKRBT),and evaluate the efficacy, safety, indication,and complications of Holmium YAG laser for the treatment of bladder carcinoma comparing with the plasmakinetic technique.Method 110 patients of bladder carcinoma were divided into two groups randomly,selected from patients in Departmen of Urology of Dandong Central Hospital from July 2009 to March 2010.Among 110 cases,55 were treated with HOBLT,and the others,PKRBT.Different clinical aspects of the patients and tumours were considered.The significant markers in both groups were recorded and evaluated,included the situation of pre-operation,intra-operation and postoperation.The pre-operation consists of the general records of patients'medical background,hemanalysis,blood biochemical analysis,and the position,amount,pathology of the tumor,the pre-operation consists of total operative duration,exfoliative cytological examination of urine,and the post-operation consists of biopsy of the wound surface,the time of postoperative bladder irrigation and catheterization,the longth of stay,and complications.Result The patients in both groups had comparable characteristics before surgery.No significant difference between the two groups was found in statistics.Every operate of the patients in both groups went smoothly and off very well,no blood transfusion and no perforation of bladder occurred. The index of hemanalysis and blood biochemical analysis are all within normal limits. The mean time of gross hematuria is less after operation with HOLBT(2.8±1.1d) than that with PKRBT(3.1±2.5),and marked difference is found between the two groups(P<0.01).Bladder irrigation (13.6±12.9h), catheter duration(4.1±1.8d),and hospital stay(6.0±1.6d) is less for HOLBT than PKRBT(21.1±17.3h,5.3±2.1d,6.9±1.9d,repectively),and significant differences are found between the two groups (P<0.05).There is no significant difference between the two groups in operative duration and complications(P>0.05).Conclusion The HOLBT technique can be used safely and effectively in treating bladder tumors.HOLBT is effective and safe as PKRBT, however, with less obtaining of adverse side-effect recovering time.it has many well-developed applications, and should illustrate a good prospect of application and extension. |
