Objective:Compared the clinical effects of the unilateral or bilateral open-door surgery through cervical posterior approach, which measured the spinal canal width before,with those of without measuring the spinal canal width before to explore the clinical significance of measuring the spinal canal width before the unilateral or bilateral open-door surgery through cervical posterior approach.Methods:Analysed the statistics of 46 patients with cervical spondylotic myelo-pathy,who were treated in the first affiliated hospital of Dalian Medical University during 2008-2010.There were 36 male cases and 14 female cases.The age varied from 29 years old to 74 years old, and the average age was 60.2 years old. The patients were divided into 2 groups. The operations of one group were strictly carried out under imaging, while the other group adopted traditional surgery.Then evaluate the improvement of spinal function after surgeries under JOA of Japaneses Orthopaedoc Association to analyse the effcts and releated factors.Results:(1)the improvement rate of the surgery measuring the spinal canal width before was 63.9%, while that of the traditional surgery was 61.6%.(2) the fineness rate of the surgery measuring the spinal canal width before was 88.9%while that of the traditional surgery was 89.3%.(3)According to the analysis of u test, there were significant differences between the surgery measuring the spinal canal width before and the traditional surgery(P<0.05). Conclusion:(1)The clinical effects of surgery according to the spinal canal width which measurd before was better than open-door surgery according to traditional width (open the door close to the inner margin of articular process).(2)Because of individual differences in age,sex,stature and weighth, as well as it is easy to form ossification in the process of the disease, there may be inaccuracy when open the door close to the inner margin of articular process, which may leads to an unsuitbale size of the door. We should choose imagings (CT) which have obviouse bone substance to measure the width of C3-C7 in the horizontal plane in which the two vertebroartery foramen was significant. Take down the data and use it in the surgery, which could make the surgery more accurate and the clinical effects more obviouse.(3)The application was suitable.According to different individual inform-ations of patients, we should choose different surgery methods.
